Decision-analytic models are increasingly developed to simulate disease burden and interventions in different settings in order to evaluate the benefits and cost-effectiveness of primary and secondary interventions. This article is a review of mathematical models that have been used to evaluate HPV vaccination in the context of developed countries with existing screening programs. Despite variations in model assumptions and uncertainty in existing data, pre-adolescent vaccination of females in the setting of current screening practices has…

The burden of disease in patients with personality disorders was investigated using a generic quality of life measure. In a sample of 1,708 individuals with a clinical diagnoses of personality disorders the EuroQol EQ-5D (EQ-5D) was administered. The results showed that the mean EQ-5D score was 0.56 and that the total number of personality disorder diagnoses rather than the specific type of disorder determined the quality of life. The borderline personality disorder was not associated…

ReviewPublication 2008Cost-Effectiveness of Vaccination: Review of Modelling Approaches
This review examines the modelling approaches used in cost-effectiveness analyses (CEAs) of vaccination programs. After …
This review examines the modelling approaches used in cost-effectiveness analyses (CEAs) of vaccination programs. After overviewing the key attributes of models used in CEAs, a framework for categorizing theoretical models is presented. Categories are based on three main attributes: static/dynamic; stochastic/deterministic; and aggregate/individual based.

ArticlePublication 2006Can Discrete Event Simulation be of Use in Modeling Major Depression?
This article, published in Cost Effectiveness and Resource Allocation, reviews the published literature on Markov …
This article, published in Cost Effectiveness and Resource Allocation, reviews the published literature on Markov models in depression and identified potential limitations in using this particular modelling approach in this disease area. Additionally, the authors develop a “Discrete Event Simulation” (DES) model to investigate the benefits and drawbacks of this simulation method compared with Markov modelling techniques. The findings of this study indicate that the most important limitation of using Markov models in depression is…

ReviewPublication 2005Implications of Modern Decision Science for Military Decision-Support Systems
This review monograph was prepared in response to a request from the United States Air …
This review monograph was prepared in response to a request from the United States Air Force Research Laboratory for a study of modern decision science that would aid in its planning of research programs and, more specifically, developing methods and tools for decision support. The emphasis is on relatively high-level decisionmaking rather than, say, that of pilots or intelligence analysts in the midst of real-time operations. They focus largely on what the military refers to as…

This article presents an epidemiologic model of hepatitis C in the United States. The authors …
This article presents an epidemiologic model of hepatitis C in the United States. The authors used empirical calibration of model parameters to gain insights into uncertainty in the natural history of hepatitis C and to improve future projections. The authors identified model inputs by way of a systematic review. Model simulations were conducted and model predictions were compared with epidemiologic data on infection prevalence and mortality from liver cancer. Goodness-of-fit criteria were used to identify…

ReviewPublication 2001Modeling for Health Care and Other Policy Decisions: Uses, Roles and Validity
This is a review article of the role of modeling approaches to guide decision making …
This is a review article of the role of modeling approaches to guide decision making in health care and other domains. The role of models to support recommendations on the cost-effective use of medical technologies and pharmaceuticals is controversial. At the heart of the controversy is the degree to which experimental or other empirical evidence should be required prior to model use. The authors argue that the controversy stems in part from a misconception that…

The limitations of diagnostic "accuracy" as a measure of decision performance require introduction of the concepts of the "sensitivity" and "specificity" yet even these do not provide a unique description of diagnostic performance because they depend on the arbitrary selection of a decision threshold. The receiver operating characteristic (ROC) curve is shown to be a simple yet complete empirical description of this decision threshold effect, indicating all possible combinations of the relative frequencies of the…
