This extended cost-effectiveness analysis (ECEA) evaluates the non-health benefits of rotavirus vaccination in Malaysia from the household’s perspective. The authors found that rotavirus vaccination reduces rotavirus episodes and expenditure substantially and provides financial risk protection to all income groups. Although the rich are paying more out of pocket than the poor by utilizing more expensive healthcare, the poor are paying more in proportion to household income. Poverty reduction benefits are concentrated amongst the poorest two…

This article investigates two cases of priority setting to explore how, in addition to technical considerations, ethical and political factors shape the allocation of health resources. First, they discuss how Thai authorities adjudicated a coverage decision for HLA-B*1502 screening, which meets the national cost-effectiveness threshold for only some of the conditions it can detect. Second, they consider England’s Cancer Drugs Fund to investigate the interplay of technical decision making and political reality. The findings suggest four concluding…

Responding to the economic and health burden of noncommunicable diseases (NCDs), the World Health Organization (WHO) introduced the Package of Essential Noncommunicable disease (PEN) interventions. Several countries, including Indonesia, implemented the PEN program. To assess the value of the investment in the current program, an economic evaluation of the program was conducted with collaboration between the Ministry of Health in Indonesia, the WHO, and the International Decision Support Initiative (iDSI). This study evaluated the delivery of…

This chapter examines the health, economic benefits, and government costs associated with scaling up a publicly financed home-based neonatal care package in rural India. This extended cost-effectiveness analysis considers two intervention scenarios against a baseline of no home-based neonatal care, both of which would utilize community health workers - one would provide coverage to approximately 7 out of 10 rural newborns, and the other would provide coverage to 9 out of 10 rural newborns.

This report from the RAND Corporation dissects the impact of the Health Technology Assessment (HTA) program in making quality research accessible and cost-effective. The authors conduct an economic analysis to illustrate benefits of new interventions in two ways: through improved health, measured in QALYs, and through showing that the intervention allows for the same health outcome and a lower cost. The report utilizes economic analyses and case studies to make recommendations. These recommendations include considering…

This extended cost-effectiveness analysis evaluates public financing of HPV vaccination in China to prevent cervical cancer. Authors …
This extended cost-effectiveness analysis evaluates public financing of HPV vaccination in China to prevent cervical cancer. Authors estimated the distribution of deaths averted by income quintile, compared vaccination paired with screening against current practice. They estimated reductions in cervical cancer incidence, net costs to the government, and patient cost savings, as well as the incremental government health care costs per death averted. When the cost per HPV vaccination is less than US$50 per vaccinated girl, vaccination was…

This chapter presents an approach for performing benefit-cost analysis using cleft lip and palate (CLP) repair as a model surgical intervention in a subspecialty hospital dedicated to CLP in India, and a benefit-cost analysis based on secondary data that model the benefit and cost of cesarean delivery for treatment of obstructed labor in 47 low- and middle-income countries. This chapter is from Essential Surgery, the first volume in the Disease Control Priorities, third edition (DCP3) series. The…

In this special issue of The Journal of the Economics of Ageing, we focus on economic aspects of population ageing in the world’s two population superpowers: China and India. China and India have been the subject of comparison for many years. Observations about their relative political and economic development abound (see for example Sen, 2013), but little analysis is currently available of their comparative demographic trajectories and the possible economic consequences of the population ageing that they are both undergoing. These demographic…

Research in areas of low skilled attendant coverage found that maternal mortality is paradoxically higher …
Research in areas of low skilled attendant coverage found that maternal mortality is paradoxically higher in women who seek obstetric care. Using unmatched population-based case-control analysis of national datasets, the authors compared the effect of health-facility admission at any time (antenatal, intrapartum, postpartum) on maternal deaths (cases) to women reporting pregnancies (controls). Probability of maternal death decreased with increasing skilled attendant coverage, among both women who were and were not admitted to a health-facility; however,…
