Policy makers in low-income and lower-middle-income countries (LMICs) are increasingly looking to develop ‘evidence-based’ frameworks for identifying priority health interventions. This paper synthesizes and appraises the literature on methodological frameworks – which incorporate economic evaluation evidence – for the purpose of setting healthcare priorities in LMICs. A systematic search of Embase, MEDLINE, Econlit and PubMed identified 3968 articles with a further 21 articles identified through manual searching. A total of 36 papers were eligible for inclusion.…

In 1993, the US Public Health Service convened a panel of 13 nongovernment scientists and scholars with expertise in economics, clinical medicine, ethics, and statistics to review the state of cost-effectiveness analysis and to develop recommendations for its conduct and use in health and medicine. Publishing their results in 1996, they proposed the most explicit set of guidelines (together with their rationale) ever defined on the conduct of CEAs. Medicare currently pays for 23 preventive services in its benefits package, the majority of which were added since 2005. In the past decade, the program has transformed from one essentially administering treatment claims, to one increasingly focused on health promotion and maintenance. What is largely unappreciated is the role cost-effectiveness analysis has played in the coverage of preventive services. This study reviews the role of cost-effectiveness analysis in Medicare coverage of preventive services and contrast it…

The use of fiscal instruments, such as taxes or subsidies, to promote healthier dietary behavior has been of increasing interest in the last decade as the evidence-base builds for the health and economic consequences of obesity, overweight, and unhealthy eating. The motivation for using fiscal instruments in nutrition policy is to make the unhealthy option less affordable and less economically attractive by increasing the price via a tax, and therefore reduce the incentive to consume…
