This paper investigates the cost-effectiveness of using rapid point-of-care tests for prenatal syphilis screening among pregnant women in sub-Saharan Africa, a region with syphilis prevalence rates as high as 17%, and where traditional multi-test screening methods have been challenging to implement. Focusing on newly available rapid point-of-care screening tests, strategies differed by the initial test [rapid plasma reagin (RPR), immunochromographic strip (ICS)], need for confirmation with Treponema pallidum hemagglutination assay, and number of visits required.…

The open-access book, "Handbook of Applied Health Economics in Vaccines," looks at the complexities of vaccine discovery, financing, and distribution. It highlights the inadequacy of standard economic models for vaccines. The book explores alternative principles challenging market-based approaches and equips readers with tools for assessing costs and benefits through practical exercises. It serves as a comprehensive resource for decision-making in vaccine development and distribution and emphasizes the importance of considering broader perspectives beyond economic efficiency.…

ArticlePublication 2022Comparative Health Systems Analysis of Differences in Catastrophic Health Expenditure

The growing burden of non-communicable diseases (NCDs) in low- and middle-income countries may have implications for health system performance in the area of financial risk protection, as measured by catastrophic health expenditure (CHE). This article compares non-communicable diseases catastrophic health expenditure to the CHE cases caused by communicable diseases across health systems to examine whether: (1) disease burden and catastrophic health expenditure are linked, (2) Catastrophic health expenditures secondary to NCDs disproportionately affect wealthier households and (3) whether the drivers…

In this paper, the authors describe the differences in population level outcomes compared to individual patients and discuss ways that these are differences. The authors cover topics including the difference between relative and absolute risk and benefit. They use an example of the decision to start anticoagulation in new-onset atrial fibrillation to discuss translating population level evidence to treatment of an individual. These options include generalizability, subgroup analysis, prediction rules, following response to therapy, and even…

Motivated by the need for evidence on cardiovascular disease (CVD) interventions offering good value for the money, the authors conducted a systematic review, including 50 studies. Included studies were those that reported full economic evaluations of individual and population-based interventions (pharmacologic and non-pharmacologic), for primary and secondary prevention of CVD among adults in LMIC. The majority of the studies were of modelled evaluations, with significant heterogeneity in methods. Most of the economic evaluations evaluated were…

In 2016 the World Health Assembly adopted the Global Strategy on Sexually Transmitted Infections (STI) 2016–2021 aiming to reduce curable STIs by 90% by 2030. This analysis costed scaling-up priority interventions to achieve coverage targets. Strategy-targeted declines in Chlamydia trachomatis, Neisseria gonorrhoeae, Treponema pallidum and Trichomonas vaginalis were applied to WHO-estimated regional burdens at 2012 levels. Case management was costed for the curable STIs, symptomatic Herpes Simplex Virus 2 (HSV-2), and non-STI vaginal syndromes, with incrementally expanding diagnoses. Service…

This article provides an overview of the main recommendations of the 2016 Second Panel on Cost-Effectiveness in Health and Medicine. In 1993, the U.S. Public Health Service convened the first panel of experts to review the state of cost-effectiveness analysis and to develop guidelines for its use in health, to improve quality and promote comparability. Scientists and scholars in economics, clinical medicine, ethics, and statistics met to share expertise and develop recommendations by consensus. The…

ArticlePublication 2015Broader Economic Impact of Vaccination: Reviewing and Appraising the Strength of Evidence
Economic evaluations of public health programs such as immunization often consider only direct health benefits and …
Economic evaluations of public health programs such as immunization often consider only direct health benefits and medical cost savings. Evidence linking immunization to important benefits in indicators such as childhood development, household behavior, and other macro-economic data are unclear. A conceptual framework of the pathways between immunization and these broader economic benefits was developed through expert consultation. The authors obtained articles from previous reviews, snowballing, and expert consultation, and associated them with one of the pathways and assessed them using modified Grading…
