The US Environmental Protection Agency (EPA) Guidelines for Preparing Economic Analyses provide a framework for assessing the impacts of environmental regulations and policies that has been extensively peer-reviewed and is widely-applied both within and outside of the agency. The Guidelines discuss: (1) statutory and executive order requirements for conducting economic analyses; (2) identifying the need for policy action; (3) regulatory and non-regulatory approaches to pollution control; (4) baseline definition; (5) discounting future benefits and costs;…

This article, written for the non-decision analyst, describes what decision analysis is, what it can and cannot do, why one should care to do this, and how one does it. Keeney describes decision analysis as "a formalization of common sense for decision problems which are too complex for informal use of common sense." He provides a more technical definition also, describing decision analysis as "a philosophy, articulated by a set of logical axioms, and a methodology…

Policy makers in low-income and lower-middle-income countries (LMICs) are increasingly looking to develop ‘evidence-based’ frameworks for identifying priority health interventions. This paper synthesizes and appraises the literature on methodological frameworks – which incorporate economic evaluation evidence – for the purpose of setting healthcare priorities in LMICs. A systematic search of Embase, MEDLINE, Econlit and PubMed identified 3968 articles with a further 21 articles identified through manual searching. A total of 36 papers were eligible for inclusion.…

This is a review of economic evaluations of complex health interventions. Complex interventions, involving interlinked packages of care, challenge the application of current methods of economic evaluation that focus on measuring only health gain. The authors find that complex interventions may be problematic on two levels. First, the complexity means the intervention may not fit into one of the current appraisal systems, and/or second, maximizing health is not the only objective. This paper discusses the…

Ethical dilemmas arising in setting priorities among interventions and among individuals in need of care …
Ethical dilemmas arising in setting priorities among interventions and among individuals in need of care are most acute when needs are great and resources few. This chapter from the Disease Control Priorities in Developing Countries 2nd edition addresses some of these concerns, identifying some of the principal ethical issues that arise in the development and allocation of effective interventions for developing countries and discussing some alternative resolutions. Resource allocation in health and elsewhere should satisfy two main…

This perspective anticipates the availability of serologic antibody testing and considers its potential usefulness in mitigation policy to reduce COVID-19 transmission. For example: Could we screen for serologic antibodies as a proxy for possible immunity and identify people who could return to the workplace with less severe mitigation measures? The authors acknowledge the uncertainties raised by many policy actors, including the WHO, such as, "Do antibodies confer immunity and, if so, for how long? How accurate is…

This “Frequently Asked Question” style document gives a brief overview of Regulatory Impact Analysis (RIA), a method used by the US government to assess the consequences of proposed policies and their alternatives. The document is aimed at those who are new to the topic. RIAs apply a framework for collecting, organizing, and evaluating data. The goal of this American Journal of Preventive Medicine supplement on “The Use of Economics in Informing U.S. Public Health Policy” is to influence policy researchers to identify and undertake economic research that generates the key evidence needed to inform policy. In public health, economic evaluation, primarily cost and cost-effectiveness analysis, has been widely used to demonstrate the economic burden of health-related conditions and the value of proposed programs and policies. However, despite the wealth…
