This chapter presents an approach for performing benefit-cost analysis using cleft lip and palate (CLP) repair as a model surgical intervention in a subspecialty hospital dedicated to CLP in India, and a benefit-cost analysis based on secondary data that model the benefit and cost of cesarean delivery for treatment of obstructed labor in 47 low- and middle-income countries. This chapter is from Essential Surgery, the first volume in the Disease Control Priorities, third edition (DCP3) series. The…

This report provides recommendations for conducting a Budget Impact Analysis from the ISPOR 2012 Budget Impact Analysis Good Practice II Task Force. National regulatory agencies such as the National Institute for Health and Clinical Excellence in England and Wales and the Pharmaceutical Benefits Advisory Committee in Australia, as well as managed care organizations in the United States, now require that companies submit estimates of both the cost-effectiveness and the likely impact of the new health-care…

In the past decade, “big push” global health initiatives financed by international donors have aimed to rapidly reach ambitious health targets in low-income countries. The health system impacts of these efforts are infrequently assessed. Saving Mothers, Giving Life is a global public-private partnership that aims to reduce maternal mortality dramatically in one year in eight districts from Uganda and Zambia. The authors evaluate the first six to twelve months of the program’s implementation, its ownership…

This lesson plan from the Centers for Disease Control and Prevention (CDC) was curated to teach high school students, grades 9-10, to explore decision-making around natural disaster emergencies. The lesson includes a video, public health emergency response data, and a response plan. It explores interactive cases of cholera in Haiti after an earthquake and breaks down risk factors and different approaches to the scenario. Students make informed decisions based on this data, exploring public health…

Resource allocation is a challenging issue faced by health policy decision makers requiring careful consideration of many factors. The aim of this study was to analyse the peer-reviewed literature to identify criteria reported in empirical studies that involved healthcare decisionmakers and in studies describing multicriteria tools.

This book explores the “hidden forces” that shape decisions as an argument against the common assumption that people act in fundamentally rational ways. From drinking coffee to losing weight, people consistently overpay, underestimate, and procrastinate. Ariely shows that such misguided behaviors are systematic and predictable or “predictably irrational.”

The burden of disease in patients with personality disorders was investigated using a generic quality of life measure. In a sample of 1,708 individuals with a clinical diagnoses of personality disorders the EuroQol EQ-5D (EQ-5D) was administered. The results showed that the mean EQ-5D score was 0.56 and that the total number of personality disorder diagnoses rather than the specific type of disorder determined the quality of life. The borderline personality disorder was not associated…

This paper applies a decision analytic approach to a past decision made by Benjamin Franklin with regard to inoculating his son against smallpox. Benjamin Franklin in his autobiography said: “In 1736 I lost one of my sons, a fine boy of four years old, by the smallpox taken in the common way. I long regretted bitterly and still regret that I had not given it to him by inoculation. This I mention for the sake of the parents who…

The limitations of diagnostic "accuracy" as a measure of decision performance require introduction of the concepts of the "sensitivity" and "specificity" yet even these do not provide a unique description of diagnostic performance because they depend on the arbitrary selection of a decision threshold. The receiver operating characteristic (ROC) curve is shown to be a simple yet complete empirical description of this decision threshold effect, indicating all possible combinations of the relative frequencies of the…
