Many low- and middle-income countries now aspire to universal health coverage, where governments ensure that all people have access to the quality health services they need without risk of impoverishment. But for universal health coverage to become reality, the health services offered must be consistent with the funds available-and this implies tough everyday choices for policymakers. This article, written for the non-decision analyst, describes what decision analysis is, what it can and cannot do, why one should care to do this, and how one does it. Keeney describes decision analysis as "a formalization of common sense for decision problems which are too complex for informal use of common sense." He provides a more technical definition also, describing decision analysis as "a philosophy, articulated by a set of logical axioms, and a methodology…

In 1993, the US Public Health Service convened a panel of 13 nongovernment scientists and scholars with expertise in economics, clinical medicine, ethics, and statistics to review the state of cost-effectiveness analysis and to develop recommendations for its conduct and use in health and medicine.
