This extended cost-effectiveness analysis (ECEA) evaluates the non-health benefits of rotavirus vaccination in Malaysia from the household’s perspective. The authors found that rotavirus vaccination reduces rotavirus episodes and expenditure substantially and provides financial risk protection to all income groups. Although the rich are paying more out of pocket than the poor by utilizing more expensive healthcare, the poor are paying more in proportion to household income. Poverty reduction benefits are concentrated amongst the poorest two…

This article investigates two cases of priority setting to explore how, in addition to technical considerations, ethical and political factors shape the allocation of health resources. First, they discuss how Thai authorities adjudicated a coverage decision for HLA-B*1502 screening, which meets the national cost-effectiveness threshold for only some of the conditions it can detect. Second, they consider England’s Cancer Drugs Fund to investigate the interplay of technical decision making and political reality. The findings suggest four concluding…

Medicare currently pays for 23 preventive services in its benefits package, the majority of which were added since 2005. In the past decade, the program has transformed from one essentially administering treatment claims, to one increasingly focused on health promotion and maintenance. What is largely unappreciated is the role cost-effectiveness analysis has played in the coverage of preventive services. This study reviews the role of cost-effectiveness analysis in Medicare coverage of preventive services and contrast it…

This is a review of economic evaluations of complex health interventions. Complex interventions, involving interlinked packages of care, challenge the application of current methods of economic evaluation that focus on measuring only health gain. The authors find that complex interventions may be problematic on two levels. First, the complexity means the intervention may not fit into one of the current appraisal systems, and/or second, maximizing health is not the only objective. This paper discusses the…

Over the past two years of the COVID-19 pandemic, the scientific community has mounted significant efforts to combat the highly transmissible SARS-CoV-2 virus. Despite substantial progress in vaccines and treatments, leading to lower hospitalization and death rates, the virus has continued to evolve, primarily through mutations. From the onset, diagnostic tests have been crucial in identifying and controlling the virus spread. The scientific world has pioneered various diagnostic techniques, including nucleic acid, antigen, and antibody-based…

Diagnostics are essential in managing the COVID-19 pandemic, with three primary methods for detecting SARS-CoV-2 infections. Each test type remains vital as we shift from responding to controlling the pandemic. Molecular tests, like PCR, are highly precise in identifying viral RNA and are endorsed by the WHO for confirming cases and guiding public health actions. Antigen rapid tests spot viral proteins and, while less accurate than molecular tests, are quicker, more affordable, simpler, and can…

During a pandemic, decisions must be made under rapidly changing, uncertain conditions. Despite advances in analytical methods for gaining early situational awareness (i.e., of a disease’s transmissibility and severity) and for predicting the likely effectiveness of interventions, a major gap exists globally in terms of integrating this information in policy documents. The authors argue that mathematical and statistical models are important tools for pandemic planning and response. Once an outbreak of pandemic potential has been…

Widespread acceptance of a vaccine for SARS-CoV-2 will be the next major step to fight the COVID-19 pandemic, but high uptake achievement will be a challenge and may be impeded by online misinformation. This article is a randomized controlled trial in the U.K. and the U.S. with the objective to quantify how exposure to online misinformation around COVID-19 vaccines affects intent to vaccinate to protect oneself or others. The authors report that in both countries…

The COVID-19 pandemic has powerfully demonstrated the critical need for public health protections. Misinformation and conflicting messages about COVID-19 have confused Americans and eroded trust in public health organizations. Maintaining and strengthening the credibility of these organizations at the federal, state, and local levels is critical to making communities healthier and ultimately saving lives. The Public Health Communications Collaborative (PHCC) was formed in 2020 to coordinate and amplify public health messaging on COVID-19 and increase…
