This paper investigates the cost-effectiveness of using rapid point-of-care tests for prenatal syphilis screening among pregnant women in sub-Saharan Africa, a region with syphilis prevalence rates as high as 17%, and where traditional multi-test screening methods have been challenging to implement. Focusing on newly available rapid point-of-care screening tests, strategies differed by the initial test [rapid plasma reagin (RPR), immunochromographic strip (ICS)], need for confirmation with Treponema pallidum hemagglutination assay, and number of visits required.…

ArticlePublication 2022Estimated Transmission Outcomes and Costs of SARS-CoV-2 Diagnostic Testing, Screening, and Surveillance Strategies Among a Simulated Population of Primary School Students

In the wake of the COVID-19 pandemic's significant educational disruptions, the U.S. government allocated $10 billion in March 2021 for testing in schools. The study aimed to analyze the costs and benefits of different COVID-19 testing strategies, particularly focusing on full-time, in-person elementary and middle school education. Utilizing an updated agent-based network model, the study simulated transmission scenarios in schools, considering various testing strategies ranging from diagnostic testing (test-to-stay) to reduce symptom-based isolations, routine screening…

ArticlePublication 2022Modeling the Effectiveness of Healthcare Personnel Reactive Testing and Screening for the SARS-Cov-2 Omicron Variant within Nursing Homes
The Omicron variant of SARS-CoV-2 has been hypothesized to exhibit faster clearance (time from peak …
The Omicron variant of SARS-CoV-2 has been hypothesized to exhibit faster clearance (time from peak viral concentration to clearance of acute infection), may be less detectable by antigen tests (decreased sensitivity), and may be more likely to evade immunity conferred by past infection or vaccination) compared to prior variants. This calls for a reassessment of strategies to prevent its spread, especially in vulnerable settings like nursing homes. Using a model that tracks viral shedding patterns,…

ArticlePublication 2021New Microsimulation Models to Inform Cervical Cancer Control
Health decision models consider the lifetime natural history of human papillomavirus (HPV) infection and pathogenesis …
Health decision models consider the lifetime natural history of human papillomavirus (HPV) infection and pathogenesis of cervical cancer, and estimate the long-term impact of preventive interventions. We propose a new health decision modeling framework that de-emphasizes previously used cytologic-colposcopic-histologic diagnoses, which are subjective and lack reproducibility, relying instead on HPV type and duration of infection as the major determinants of model transition probabilities. We posit that new model health states and corollary transitions are universal,…

ArticlePublication 2022Early HPV Natural History Transitions
Microsimulation models used to evaluate the cost-effectiveness of novel cervical cancer screening technologies rely on …
Microsimulation models used to evaluate the cost-effectiveness of novel cervical cancer screening technologies rely on accurate transition risks for human papillomavirus (HPV) infection, persistence (or absence of HPV clearance), progression to precancerous lesions, and invasion. To inform the refinement of such models, we compared the early natural history of HPV types using prospective data from immunocompetent women in the Guanacaste Natural History Study, the ASCUS-LSIL Triage Study, and the Costa Rica HPV Vaccine Trial. We…

This resource pack, curated by the Center for Health Decision Science, is a collection of models of HPV-related cervical cancer, differing in design, structure and features based on analytic objectives. In many ways, HPV and its related diseases represent a prototypical public health problem given the communicable and non-communicable nature of disease, opportunities for intervention along the entire disease spectrum (e.g., primary and secondary prevention, diagnosis, treatment), the varied ages at which interventions are targeted…

The Millennium Mathematics Project (MMP), a collaboration between the Faculties of Mathematics and Education at the University of Cambridge, is a maths education and outreach initiative for ages 3 to 19 and the general public. The focus is on increasing mathematical understanding, confidence and enjoyment, developing problem-solving skills, and promoting creative and imaginative approaches to maths. The project consists of a family of complementary programmes, including the NRICH website, Plus online mathematics magazine, Wild Maths, and…

Testing for and treating latent tuberculosis infection (LTBI) is among the main strategies to achieve TB elimination in the United States. This analysis estimated health outcomes, costs, and cost-effectiveness of LTBI testing and treatment among non-US born residents with and without medical comorbidities (e.g., diabetes, HIV infection, and end-stage renal disease). A decision analytic tree and Markov cohort simulation model was used to compare the following strategies: no testing, tuberculin skin test (TST), interferon gamma release assay…

This report from RAND reviews decision-support tools, including models and nonmodeling approaches, that are relevant to infectious disease prevention, detection, and response and aligns these tools with real-world policy questions that the tools can help address. This overview is designed to help modelers and other technical experts understand the questions that policymakers will raise and the decisions they must make. The report also presents policymakers with the capabilities and limitations of the different tools that may…
