ICER is a non-profit organization that evaluates evidence on a range of topics including the value of medical tests, treatments and delivery system innovations and moves that evidence into action to improve the health care system. To accomplish this goal ICER performs analyses on effectiveness and costs, supports specific programs, and develops reports using innovative methods that make it easier to translate evidence into decisions that can align efforts to use evidence to drive improvements in both…

Policy makers in low-income and lower-middle-income countries (LMICs) are increasingly looking to develop ‘evidence-based’ frameworks for identifying priority health interventions. This paper synthesizes and appraises the literature on methodological frameworks – which incorporate economic evaluation evidence – for the purpose of setting healthcare priorities in LMICs. A systematic search of Embase, MEDLINE, Econlit and PubMed identified 3968 articles with a further 21 articles identified through manual searching. A total of 36 papers were eligible for inclusion.…

BookPublication 2014Decision Making in Health and Medicine: Integrating Evidence and Values
Decision making in health care involves consideration of a complex set of diagnostic, therapeutic and …
Decision making in health care involves consideration of a complex set of diagnostic, therapeutic and prognostic uncertainties. Medical therapies have side effects, surgical interventions may lead to complications, and diagnostic tests can produce misleading results. Furthermore, patient values and service costs must be considered. Decisions in clinical and health policy require careful weighing of risks and benefits and are commonly a trade-off of competing objectives: maximizing quality of life vs maximizing life expectancy vs minimizing…

The use of fiscal instruments, such as taxes or subsidies, to promote healthier dietary behavior …
The use of fiscal instruments, such as taxes or subsidies, to promote healthier dietary behavior has been of increasing interest in the last decade as the evidence-base builds for the health and economic consequences of obesity, overweight, and unhealthy eating. The motivation for using fiscal instruments in nutrition policy is to make the unhealthy option less affordable and less economically attractive by increasing the price via a tax, and therefore reduce the incentive to consume…

Responding to the economic and health burden of noncommunicable diseases (NCDs), the World Health Organization (WHO) introduced the Package of Essential Noncommunicable disease (PEN) interventions. Several countries, including Indonesia, implemented the PEN program. To assess the value of the investment in the current program, an economic evaluation of the program was conducted with collaboration between the Ministry of Health in Indonesia, the WHO, and the International Decision Support Initiative (iDSI). Cost-effectiveness analysis assumes a single constraint, in the form of the budget constraint, whereas in reality decision makers may be faced with numerous other constraints. The objective of this article is to develop a typology of constraints that may act as barriers to implementation of cost-effectiveness recommendations. Six categories of constraints are considered: the design of the health system; costs of implementing change; system interactions between interventions; uncertainty in estimates of costs and benefits; weak governance;…

The recommendations are based on the authors' own experiences and so are practical in nature. The book describes the challenges of allocating resources efficiently and fairly, including challenges in methodology and policy formation. Examples are provided.…

This text was conceived and developed in the Harvard T.H. Chan School of Public Health at the Center for the Analysis of Health Practices. The book had its origins in a set of classroom materials developed during the academic year 1974-75 for an elective course in medical decision making at the Harvard Medical School. In this book students are shown how to structure clinical decision problems, how to systematically formulate the intertwining roles of diagnosis and treatment, how to…
