The aim of cost-utility analysis is to support decision making in healthcare by providing a standardized mechanism for comparing resource use and health outcomes across programmes of work. The focus of this review is the denominator of the cost-utility analysis, specifically the methodology and statistical challenges associated with calculating QALYs from patient-level data collected as part of a trial. The authors provide a brief description of the most common questionnaire used to calculate patient level…

Long-acting antiretroviral therapy (LA-ART) could potentially improve outcomes in particular for those HIV-infected individuals with poor daily ART adherence. In this study the clinical role and cost-effectiveness of 3 long-acting ART strategies (LA-ART for patients with multiple ART failures; second-line LA-ART for those failing first-line therapy; and first-line LA-ART for ART-naive patients) are compared to daily oral ART using a microsimulation model. The findings of this study showed that LA-ART could improve survival of…

ArticlePublication 2015Publicly Financed HPV Vaccination in China: Extended Cost-Effectiveness Analysis
This extended cost-effectiveness analysis evaluates public financing of HPV vaccination in China to prevent cervical cancer. Authors …
This extended cost-effectiveness analysis evaluates public financing of HPV vaccination in China to prevent cervical cancer. Authors estimated the distribution of deaths averted by income quintile, compared vaccination paired with screening against current practice. They estimated reductions in cervical cancer incidence, net costs to the government, and patient cost savings, as well as the incremental government health care costs per death averted. When the cost per HPV vaccination is less than US$50 per vaccinated girl, vaccination was…

Using extended cost-effectiveness analysis (ECEA), this paper evaluates the health and economic implications of different vaccine delivery strategies in Ethiopia: (1) routine immunization, (2) routine immunization with financial incentives, and (3) mass campaigns, known as supplemental immunization activities (SIAs), for measles vaccination. At higher costs, SIAs reached higher levels of vaccine coverage. Routine immunization paired with financial incentives was found to increase the demand among poorer households.

ArticlePublication 2014Economic Implications of Population Ageing in China & India: Introduction to the Special Issue
In this special issue of The Journal of the Economics of Ageing, we focus on economic …
In this special issue of The Journal of the Economics of Ageing, we focus on economic aspects of population ageing in the world’s two population superpowers: China and India. China and India have been the subject of comparison for many years. Observations about their relative political and economic development abound (see for example Sen, 2013), but little analysis is currently available of their comparative demographic trajectories and the possible economic consequences of the population ageing that they are both undergoing. These demographic…

ArticlePublication 2014HIV Cure Strategies: How Good Must They be to Improve on ART?
This article uses a microsimulation model to explore under what circumstances three HIV cure strategies …
This article uses a microsimulation model to explore under what circumstances three HIV cure strategies (gene therapy, chemotherapy and stem cell transplantation (SCT)) would improve efficacy, toxicity, cost and quality of life outcomes compared to current antiretroviral therapy (ART). Assuming a cost-effectiveness threshold of $100,000 per QALY, results show that gene therapy was cost-effective compared to ART with an efficacy of 10%, relapse rate of 0.5%/month, and cost of $54,000. Chemotherapy was cost-effective with an…

This report provides recommendations for conducting a Budget Impact Analysis from the ISPOR 2012 Budget Impact Analysis Good Practice II Task Force. National regulatory agencies such as the National Institute for Health and Clinical Excellence in England and Wales and the Pharmaceutical Benefits Advisory Committee in Australia, as well as managed care organizations in the United States, now require that companies submit estimates of both the cost-effectiveness and the likely impact of the new health-care…

Vaccination has led to remarkable health gains over the last century. However, large coverage gaps remain, which will require significant financial resources and political will to address. In recent years, a compelling line of inquiry has established the economic benefits of health, at both the individual and aggregate levels. Most existing economic evaluations of particular health interventions fail to account for this new research, leading to potentially sizable undervaluation of those interventions. In line with…

This article, published in Value in Health, describes a Markov model that compares the cost-effectiveness of alternate sequences of treatment strategies using second-generation antipsychotics (SGAs) for patients with schizophrenia. First-line treatments include one of the four SGAs: olanzapine (OLZ), risperidone (RSP), quetiapine (QTP), and ziprasidone (ZSD). Patients are able switch to another of these antipsychotics as second-line therapy, and only clozapine (CLZ) is allowed as third-line treatment. Model input parameters were obtained from the Clinical…
