This paper is an extended cost-effectiveness analysis to model the potential health and economic impacts of a salt reduction policy in South Africa. The authors used surveys and epidemiologic studies to estimate reductions in CVD resulting from lower salt intake. They calculated the average out-of-pocket (OOP) cost of CVD care and estimated the reduction in OOP expenditures and government subsidies due to the policy. Value-of-information analysis (VOI) is an analytic approach used to inform research priorities, guide clinical trial design, and provide information for decisions about reimbursement. The authors review existing web-based tools to facilitate VOI calculations. These include Sheffield Accelerated Value of Information (SAVI), the web interface to the BCEA (Bayesian Cost-Effectiveness Analysis) R package (BCEAweb), Rapid Assessment of Need for Evidence (RANE), and Value of Information for Cardiovascular Trials and Other Comparative Research (VICTOR).

Trial-based economic evaluations are increasingly conducted to support healthcare decision-making but there are inevitable methodological challenges. This tutorial provides step-by-step guidance on how to combine appropriate statistical methods using a ready-to-use R script, and addresses missing data, correlated costs and effects, baseline imbalances, and skewness of costs and/or effects. The authors also provide the theoretical background of these methods, and demonstrate their application through a simulated trial-based economic evaluation.
