The authors use a clinical example to simulate how treatment discussions can be complicated when new evidence is introduced that conflicts with existing guidelines. Even when evidence is consistent, the authors point out that current guidelines can have interpretations that don't agree with available evidence. They develop a step-wise algorithm to help guide individual clinical decisions even in the absence of general consensus related to appropriate testing and treatment.

This article is part of a 6-part series on clinical decision making. The authors use two clinical examples to review the principles of interpreting diagnostic test results. They outline an approach that can be used to determine how to select and apply tests and their results to the practice of internal medicine. Topics covered in the two case studies include sensitivity and specificity, positive predictive and negative predictive value of tests, and how to estimate…

ToolInteractive, Teaching Resource 2020RAND Critical Care Surge Response Tool
This Excel-based model allows decisionmakers at all levels (i.e., hospitals, health care systems, states, regions) …
This Excel-based model allows decisionmakers at all levels (i.e., hospitals, health care systems, states, regions) to examine the current critical care capacity in the nation’s hospitals and rapidly explore strategies for increasing capacity to provide care for the sickest COVID-19 patients. The tool was developed by the RAND Corporation in response to the 2020 COVID-19 pandemic. Model input parameters to the Excel spreadsheet include baseline number of beds, critical care doctors and nurses, respiratory therapists,…

Resource PackPublication, Teaching Resource 2021Resource Pack: Valuing Health and Longevity in BCA
This resource pack, curated by the Center for Health Decision Science, introduces the valuation of …
This resource pack, curated by the Center for Health Decision Science, introduces the valuation of changes in health and longevity in benefit-cost analysis. It is targeted towards advanced students and practitioners who have a basic understanding of the benefit-cost analysis framework, as discussed in the Resource Pack: Introduction to Benefit-Cost Analysis. That pack provides a general overview of the approaches that are explored in more detail here, as well as related guidance documents. This pack…

Many policies aim to improve longevity, decreasing the risk of death in each year, and …
Many policies aim to improve longevity, decreasing the risk of death in each year, and the value of these risk reductions often dominate the estimated benefits of risk regulations and other policies. This value is often expressed as the value per statistical life (VSL), a term that is widely misunderstood. It is not the value that the analyst, the government, or the individual places on saving an identified life with certainty. Instead, it reflects individuals’…

ArticlePublication 2020Health Gains & Financial Risk Protection Afforded by Public Financing of Selected Malaria Interventions in Ethiopia: An ECEA
This article, published in the Malaria Journal, aims to estimate the expected health and financial …
This article, published in the Malaria Journal, aims to estimate the expected health and financial risk protection (FRP) benefits of universal public financing of key malaria interventions in Ethiopia. An extended cost-effectiveness analysis (ECEA) is used to estimate the potential health and FRP benefits of publicly financing a 10% increase in artemisinin-based combination therapy (ACT), long-lasting insecticide-treated bed nets (LLIN), indoor residual spraying (IRS), and a hypothetical malaria vaccine. The results indicate that ACT, LLIN,…

This article clarifies some misconceptions about mortality risk and economic valuation. The mortality effects of exposure to environmental hazards such as air pollution are often described by the estimated number of “premature deaths” and the economic value of an exposure reduction as the number of “statistical lives saved” multiplied by the “value per statistical life.” These terms can be misleading because the number of deaths advanced by exposure cannot be determined from mortality data; it…

Extended cost-effectiveness analyses (ECEAs) build on cost-effectiveness analyses (CEAs) by assessing consequences in both the health and non-health domains. The ECEA approach proves novel in that it includes equity and non-health benefits (FRP) in the economic evaluation of health policies, which enables multiple criteria to factor in the decision-making process. More important, the ECEA approach enables the design of benefits packages, such as essential universal health care and the highest-priority package, based on the quantitative…
