ArticlePublication 2008Mathematical Models of Cervical Cancer Prevention in Latin America and the Caribbean
This article reports on a model-based approach estimated averted cervical cancer cases and deaths, disability-adjusted …
This article reports on a model-based approach estimated averted cervical cancer cases and deaths, disability-adjusted life years (DALYs) and incremental cost-effectiveness ratios (I$/DALY averted) for human papillomavirus (HPV) vaccination of young adolescent girls using population and epidemiologic data for 33 countries in Latin America and the Caribbean (LAC). The authors found that an absolute reduction in lifetime cancer risk varied between countries, depending on incidence, proportion attributable to HPV-16 and 18, and population age-structure; for…

ArticlePublication 2008Economic Burden of Personality Disorders in Mental Health Care
This paper aimed to investigate the economic burden of patients with personality disorders in mental …
This paper aimed to investigate the economic burden of patients with personality disorders in mental health care. The direct and indirect costs were assessed for 1740 study participants with a clinical diagnosis of personality disorders using the Trimbos and Institute for Medical Technology Assessment Questionnaire on Costs Associated with Psychiatric Illness. Results indicated that the mean total costs in the 12 months prior to treatment were €11,126 per patient. Two thirds (66.5%) of these costs consisted…

ArticlePublication 2007Economic Evaluation of Hepatitis B Vaccination in Low-Income Countries: Cost-Effectiveness Affordability Curves
In the face of uncertainties about both the health and economic consequences of a vaccine …
In the face of uncertainties about both the health and economic consequences of a vaccine program, as well as the availability and magnitude of resources needed to fund the program, cost-effectiveness affordability curves can provide information to decision-makers about the probability that a program will be both cost-effective and affordable: these are distinct but equally relevant considerations in resource-poor settings. This paper describes the application of this method to assess a hepatitis B vaccination program in the…

Decision analysis consists of a prescriptive theory and associated models and tools that aid individuals …
Decision analysis consists of a prescriptive theory and associated models and tools that aid individuals or groups confronted with complex decision problems in a wide variety of contexts. Decision analysis can be applied to the environment, health and medicine, engineering, public policy, and business. This article, published in PloS One, uses maternal mortality data from Mexico to examine trends in the provision of maternal health services and the trajectory towards reaching the Millennium Development Goal 5 (MDG 5). The authors developed a model of the natural history of pregnancy and pregnancy related complications and simulated a cohort of 15-year-old women over their lifetime based on national data. The model produced clinical outcomes, costs, and cost-effectiveness of the current standard…

ReviewPublication 2006Ethical Issues in Resource Allocation, Research, and New Product Development
Ethical dilemmas arising in setting priorities among interventions and among individuals in need of care …
Ethical dilemmas arising in setting priorities among interventions and among individuals in need of care are most acute when needs are great and resources few. This chapter from the Disease Control Priorities in Developing Countries 2nd edition addresses some of these concerns, identifying some of the principal ethical issues that arise in the development and allocation of effective interventions for developing countries and discussing some alternative resolutions. Resource allocation in health and elsewhere should satisfy two main…

ReviewPublication 2001Modeling for Health Care and Other Policy Decisions: Uses, Roles and Validity
This is a review article of the role of modeling approaches to guide decision making …
This is a review article of the role of modeling approaches to guide decision making in health care and other domains. The role of models to support recommendations on the cost-effective use of medical technologies and pharmaceuticals is controversial. At the heart of the controversy is the degree to which experimental or other empirical evidence should be required prior to model use. The authors argue that the controversy stems in part from a misconception that…

This text was conceived and developed in the Harvard T.H. Chan School of Public Health at the Center for the Analysis of Health Practices. The book had its origins in a set of classroom materials developed during the academic year 1974-75 for an elective course in medical decision making at the Harvard Medical School. In this book students are shown how to structure clinical decision problems, how to systematically formulate the intertwining roles of diagnosis and treatment, how to…
