This is a review of economic evaluations of complex health interventions. Complex interventions, involving interlinked packages of care, challenge the application of current methods of economic evaluation that focus on measuring only health gain. The authors find that complex interventions may be problematic on two levels. First, the complexity means the intervention may not fit into one of the current appraisal systems, and/or second, maximizing health is not the only objective. This paper discusses the…

GuidelinesPublication 2013Economic Analyses to Support Decisions about HPV Vaccination in Low- and Middle-Income Countries: Consensus Report and Guide

Low- and middle-income countries need to consider economic issues such as cost-effectiveness, affordability and sustainability before introducing a program for human papillomavirus (HPV) vaccination. However, many such countries lack the technical capacity and data to conduct their own analyses. This report describes the consensus of an expert group convened by the World Health Organization, prioritizing key issues to be addressed when considering economic analyses to support HPV vaccine introduction in these countries. The expert group…

ArticlePublication 2012Health and Economic Outcomes of Interventions to Reduce Pregnancy-Related Mortality in Nigeria
This paper examines the cost-effectiveness and impact of individual and integrated packages of interventions aimed …
This paper examines the cost-effectiveness and impact of individual and integrated packages of interventions aimed to reduce maternal mortality in Nigeria, a country with extremely high maternal mortality rates. Using a previously validated model adapted to the Nigerian context, the study finds that an increase of access to family planning is the most effective individual strategy, which not only reduces pregnancy-related mortality but also proves to be economically efficient. However, relying solely on family planning…

This learning module provides lessons about health economics containing activities, worksheets, notes, and answers on a number of skills in probability calculations. Using mathematics, these exercises engage students by tackling real world issues in health decision-making. These activities are recommended to be used together but can also be used independently. They are aimed at a high school learning level, age 16 or higher.

Rotavirus infection is responsible for about 500,000 deaths annually, and the disease burden is disproportionately borne by children in low-income countries. Motivated by the global recommendation by the WHO that all countries include infant rotavirus vaccination in their national immunization programs, the objective of this analysis was to provide information on the expected health, economic and financial consequences of rotavirus vaccines in the 72 GAVI support-eligible countries. The authors synthesized population-level data from various sources (primarily from…

The field of behavioral economics helps to explain human decision making that would be deemed irrational by standard economic models. Dan Ariely begins this talk by presenting visual illusions, followed by the analogy that humans face similar illusions regarding our behavior. He focuses on how setting a “default” option can substantially influence our choices, especially when decisions are complex. He illustrates his points with examples from health and other fields. This article, published in PloS One, uses maternal mortality data from Mexico to examine trends in the provision of maternal health services and the trajectory towards reaching the Millennium Development Goal 5 (MDG 5). The authors developed a model of the natural history of pregnancy and pregnancy related complications and simulated a cohort of 15-year-old women over their lifetime based on national data. The model produced clinical outcomes, costs, and cost-effectiveness of the current standard…

Several factors are changing the landscape of cervical cancer control, including a better understanding of the natural history of human papillomavirus (HPV), reliable assays for detecting high-risk HPV infections, and a soon to be available HPV-16/18 vaccine. There are important differences in the relevant policy questions for different settings. By synthesizing and integrating the best available data, the use of modeling in a decision analytic framework can identify those factors most likely to influence outcomes,…
