This tutorial describes both the intent of extended cost-effectiveness analysis and its keys to allow easy implementation for health policy assessment. Extended cost-effectiveness analysis was developed to address health policy assessment, specifically to evaluate the health and financial consequences of public policies in four domains: (1) the health gains; (2) the financial risk protection benefits; (3) the total costs to the policy makers; and (4) the distributional benefits.

This study uses extended cost effectiveness analysis (ECEA) to analyze the impacts of universal public finance (UPF) on epilepsy, schizophrenia, and depression in India and Ethiopia. The Ethiopian government has launched a National Mental Health Strategy which explicitly recognizes the importance of an efficient, equitable scale-up of mental health care within a broader, ongoing effort to increase levels of health insurance in the general population. The inclusion of universal health coverage (UHC) as a target in the health Sustainable Development Goals speaks to its importance as both a foundational and an end goal for global health. Across the globe, countries are in varying stages of progress toward UHC. This chapter summarizes the findings of a systematic search of the cost-effectiveness literature on interventions to improve reproductive, maternal, newborn, and child health. Interventions for newborn health, treatment of febrile illness, immunization against preventable diseases, and micronutrient interventions remain among the most cost-effective and affordable. Other studies explore how to provide existing interventions using new platforms to increase outreach or decrease cost per person covered, or both. Interventions provided in the community may achieve both purposes to…

This chapter presents an approach for performing benefit-cost analysis using cleft lip and palate (CLP) repair as a model surgical intervention in a subspecialty hospital dedicated to CLP in India, and a benefit-cost analysis based on secondary data that model the benefit and cost of cesarean delivery for treatment of obstructed labor in 47 low- and middle-income countries. This chapter is from Essential Surgery, the first volume in the Disease Control Priorities, third edition (DCP3) series. The…

This review article is part of a special supplement on “Comprehensive Control of HPV Infections and Related Diseases.” At the time of its writing, prophylactic vaccination against human papillomavirus (HPV) in pre-adolescent females had been introduced in most developed countries, supported by modeled evaluations that had almost universally found vaccination of pre-adolescent females to be cost-effective. Vaccination of pre-adolescent males had been shown to be cost-effective at a cost per vaccinated individual of ~US$400-500 if…
