ArticlePublication 2018Cost-Effectiveness of Financial Incentives and Disincentives for Improving Food Purchases and Health through SNAP
This analysis estimated the health impact, costs, and cost-effectiveness of food incentives, disincentives, or restrictions …
This analysis estimated the health impact, costs, and cost-effectiveness of food incentives, disincentives, or restrictions in the Supplemental Nutrition Assistance Program (SNAP). A microsimulation model (CVD-PREDICT) was used to compare three policy interventions: (1) a 30% incentive for fruits and vegetables (F&V), (2) a 30% F&V incentive with a restriction of sugar-sweetened beverages (SSBs), and (3) a broader incentive/disincentive program for multiple foods that also preserves choice (SNAP-plus). From a societal perspective, all three scenarios…

This study uses extended cost-effectiveness analysis (ECEA) to evaluate the impact of three scenarios of …
This study uses extended cost-effectiveness analysis (ECEA) to evaluate the impact of three scenarios of publicly financed national epilepsy programs in India that provide (1) first line antiepilepsy drugs (AEDs), (2) first- and second-line AEDs, and (3) first- and second-line AEDs and surgery. Outcome measures include disability-adjusted life years (DALYs) averted, and cost per DALY averted. Economic benefit measures estimated include out-of-pocket (OOP) expenditure averted and money-metric value of insurance. All three scenarios represent a…

ArticlePublication 2017Catastrophic Costs Potentially Averted by TB Control in India and South Africa
This study estimated the reduction in tuberculosis-related catastrophic costs with an aggressive expansion of tuberculosis …
This study estimated the reduction in tuberculosis-related catastrophic costs with an aggressive expansion of tuberculosis services in India and South Africa from 2016 to 2035, in line with the End TB Strategy. The authors investigated three intervention scenarios: improved treatment of drug-sensitive tuberculosis; improved treatment of multidrug-resistant tuberculosis; and expansion of access to tuberculosis care through intensified case finding (South Africa only). In India and South Africa, improvements in treatment for drug-sensitive and multidrug-resistant tuberculosis…

ArticlePublication 2018Trading Bankruptcy for Health: A Discrete-Choice Experiment
This article in Value in Health evaluates the importance of improved health as compared to …
This article in Value in Health evaluates the importance of improved health as compared to improved financial risk protection in the general United States population. Using a discrete-choice experiment, it finds that 31.3% of the population values cure at all costs, and 8.5% of the population use financial solvency to dominate medical decision making. This study shares insight to the US population values and trade-offs between health outcomes and financial health, and highlights the difficult…

ArticlePublication 2016Country-Level Cost-Effectiveness Thresholds
This article estimates the cost-effectiveness thresholds (CETs) for health interventions in several low and middle-income …
This article estimates the cost-effectiveness thresholds (CETs) for health interventions in several low and middle-income countries (LMICs), based on opportunity costs. When there are constraints on a health care system’s budget or ability to increase expenditures, additional costs imposed by interventions have an “opportunity cost” in terms of the health foregone because other interventions cannot be provided. The authors argue that cost-effectiveness thresholds should reflect health opportunity cost and aim to calculate these in four…

This study sought to understand how the general public views the quality of life effects …
This study sought to understand how the general public views the quality of life effects of opioid misuse and opioid use disorder on an individual and his/her spouse, measured in terms used in economic evaluations. The study design was a cross-sectional internet survey of a US population-representative respondent panel conducted December 2013-January 2014, with a total of 2054 randomly selected adults, of whom 51.1% were male. The mean individual utility ranged from 0.574 for active injection…

Cost-effectiveness analysis assumes a single constraint, in the form of the budget constraint, whereas in reality decision makers may be faced with numerous other constraints. The objective of this article is to develop a typology of constraints that may act as barriers to implementation of cost-effectiveness recommendations. Six categories of constraints are considered: the design of the health system; costs of implementing change; system interactions between interventions; uncertainty in estimates of costs and benefits; weak governance;…

Responding to the economic and health burden of noncommunicable diseases (NCDs), the World Health Organization (WHO) introduced the Package of Essential Noncommunicable disease (PEN) interventions. Several countries, including Indonesia, implemented the PEN program. To assess the value of the investment in the current program, an economic evaluation of the program was conducted with collaboration between the Ministry of Health in Indonesia, the WHO, and the International Decision Support Initiative (iDSI). This study evaluated the delivery of…

This chapter examines the health, economic benefits, and government costs associated with scaling up a publicly financed home-based neonatal care package in rural India. This extended cost-effectiveness analysis considers two intervention scenarios against a baseline of no home-based neonatal care, both of which would utilize community health workers - one would provide coverage to approximately 7 out of 10 rural newborns, and the other would provide coverage to 9 out of 10 rural newborns.
