Medicare currently pays for 23 preventive services in its benefits package, the majority of which were added since 2005. In the past decade, the program has transformed from one essentially administering treatment claims, to one increasingly focused on health promotion and maintenance. What is largely unappreciated is the role cost-effectiveness analysis has played in the coverage of preventive services. This study reviews the role of cost-effectiveness analysis in Medicare coverage of preventive services and contrast it…

This report from the Institute of Medicine, The Swine Flu Affair: Decision-Making on a Slippery Disease, was written to review and critique the decisions made around the 1976 swine flu threat. In 1976, a small group of soldiers at Fort Dix were infected with a swine flu virus that was deemed similar to the virus responsible for the great 1918-19 world-wide flu pandemic. This systematic review examines the cost-effectiveness of newer antidiabetes medications for type 2 diabetes in the United States. The study screened nonindustry-funded cost-effectiveness analyses (CEAs) conducted from 2010 to 2023, focusing on estimating cost per quality-adjusted life-year (QALY) gained. Nine CEAs met the criteria, evaluating medications such as glucagon-like peptide-1 agonists (GLP1a), dipeptidyl peptidase-4 inhibitors (DPP4i), and sodium–glucose cotransporter-2 inhibitors (SGLT2i), among others. Comparisons were made against metformin, sulfonylureas, neutral protamine Hagedorn (NPH) insulin, and…
