This article, published in BMJ Global Health, provides experiences of country contextualization of WHO-CHOICE methods and models to a country level. Results from three contextualized cost-effectiveness analyses (CEAs) are presented, and the authors discuss how this evidence can inform priority setting in Ethiopia. This method of contextualized CEAs requires inclusion of national analysts and use of country-specific inputs for either costs, epidemiology, demography, baseline coverage or effects. Rank ordering of interventions by incremental cost-effectiveness ratios…

Based on Ethiopia’s commitment to attain universal health coverage by 2035, the authors use the current national strategies including the national essential health service package in Ethiopia as a springboard to explore the criteria and processes Ethiopia uses to set the existing national health sector priorities. Addition the authors highlight the critical need to strengthen country-led efforts and investments in human capital in developing country contexts.

The Organisation for Economic Co-operation and Development (OECD) Health Database offers a comprehensive source of comparable statistics on health and health systems across OECD countries. These data serve to facilitate and support comparative analyses, and can be useful for researchers, educators and policy makers looking to draw lessons from international comparisons of diverse health systems. General categories of data include population demographics, health status and determinants of health, as well as data on the societal responses to health…

Smoking is considered the leading risk factor for mortality among the Armenian population. The authors conducted an extended cost-effectiveness analysis (ECEA) on increases in the excise tax on cigarettes in Armenia. Based on the World Health Organization recommendations, they analyzed the impact of a 75% increase of excise tax on the retail price of cigarettes. The ECEA found large health and financial benefits to the excise tax. It averted about 88,000 premature deaths, US$63 million of OOP…

This article estimates the cost-effectiveness thresholds (CETs) for health interventions in several low and middle-income countries (LMICs), based on opportunity costs. When there are constraints on a health care system’s budget or ability to increase expenditures, additional costs imposed by interventions have an “opportunity cost” in terms of the health foregone because other interventions cannot be provided. The authors argue that cost-effectiveness thresholds should reflect health opportunity cost and aim to calculate these in four…

This is a revised and expanded edition of the original text on Cost-Effectiveness in Health and Medicine. The Second Panel on Cost-Effectiveness in Health and Medicine included experts drawn from academia, healthcare administration, and government. The book offers advice for conducting analyses to improve the allocation of health resources, and is intended for economists, policy analysts, hospital executives, and students across health, business, and humanities disciplines. Policy makers in low-income and lower-middle-income countries (LMICs) are increasingly looking to develop ‘evidence-based’ frameworks for identifying priority health interventions. This paper synthesizes and appraises the literature on methodological frameworks – which incorporate economic evaluation evidence – for the purpose of setting healthcare priorities in LMICs. A systematic search of Embase, MEDLINE, Econlit and PubMed identified 3968 articles with a further 21 articles identified through manual searching. A total of 36 papers were eligible for inclusion.…

Cost-effectiveness analysis assumes a single constraint, in the form of the budget constraint, whereas in reality decision makers may be faced with numerous other constraints. The objective of this article is to develop a typology of constraints that may act as barriers to implementation of cost-effectiveness recommendations. Six categories of constraints are considered: the design of the health system; costs of implementing change; system interactions between interventions; uncertainty in estimates of costs and benefits; weak governance;…

This article investigates two cases of priority setting to explore how, in addition to technical considerations, ethical and political factors shape the allocation of health resources. First, they discuss how Thai authorities adjudicated a coverage decision for HLA-B*1502 screening, which meets the national cost-effectiveness threshold for only some of the conditions it can detect. Second, they consider England’s Cancer Drugs Fund to investigate the interplay of technical decision making and political reality. The findings suggest four concluding…
