There is immense interest worldwide in the notion of universal health coverage. A major policy focus in moving toward universal health coverage has been on the key policy question: what services should be made available and under what conditions? This article focuses on how a feasible set of universal health coverage services can be explicitly defined to create what is commonly known as a “health benefits package”, a set of services that can be feasibly financed…

This chapter examines universal public finance of the prevention and treatment of pneumonia and diarrhea in Ethiopia, with a focus on children under age five years. This extended cost-effectiveness analysis examines benefits by income quintile so that policy makers can better understand how each package affects different segments of the population and permits the incorporation of financial risk protection in the economic evaluation of health policies - both critical elements of universal health coverage.

This chapter examines the health, economic benefits, and government costs associated with scaling up a publicly financed home-based neonatal care package in rural India. This extended cost-effectiveness analysis considers two intervention scenarios against a baseline of no home-based neonatal care, both of which would utilize community health workers - one would provide coverage to approximately 7 out of 10 rural newborns, and the other would provide coverage to 9 out of 10 rural newborns.

This article, published in Health Policy and Planning, utilizes an extended cost effectiveness analysis (ECEA) to examine how policies to expand access to surgery in rural Ethiopia would impact health, impoverishment, and equity. The study finds that health benefits, financial risk protection, and equity appear to be in tension in the expansion of access to surgical care. Health benefits from each of the examined policies accrue primarily among the poor, but without travel vouchers, many…

The burden of cardiovascular disease (CVD) and related conditions is increasing in low- and middle-income countries. Policy makers need an understanding of the magnitude and drivers of the costs of cardiovascular disease related conditions to make decisions on how to allocate limited health resources. This systematic review of the published literature documents provider-incurred costs of treatment for cardiovascular diseases and risk conditions in low- and middle-income countries. Total costs of treatment were inflated to 2012 US dollars for comparability…

Policy makers seeking to reduce childhood obesity must prioritize investment in treatment and primary prevention. The authors estimated the cost-effectiveness of seven interventions high on the obesity policy agenda: (1) a sugar-sweetened beverage excise tax, (2) elimination of the tax subsidy for advertising unhealthy food to children, (3) restaurant menu calorie labeling, (4) nutrition standards for school meals, (5) nutrition standards for all other food and beverages sold in schools, (6) improved early care and…

Countries like Vietnam transitioning to middle-income status increasingly bear the cost of both existing and new vaccines. However, the impact and cost-effectiveness of the Expanded Programme on Immunization (EPI) as a whole has never been assessed on a country level. Data on vaccine-preventable disease incidence and mortality from Vietnam's national surveillance was analyzed to estimate the likely impact that vaccination in 1980–2010 may have had. The same data were separately analyzed using the Lives Saved Tool…

The Commission on Investing in Health published its report, GlobalHealth2035, in 2013, estimating an investment case for a grand convergence in health outcomes globally. In support of the drafting of the Sustainable Development Goals (SDGs), this paper presents estimates of what the grand convergence investment case might achieve—and what investment would be required—by 2030. The authors start with a country-based (bottom-up) analysis of the costs and impact of…!--?xml:namespace prefix = "o" ns = "urn:schemas-microsoft-com:office:office" /-->

This paper is an extended cost-effectiveness analysis to model the potential health and economic impacts of a salt reduction policy in South Africa. The authors used surveys and epidemiologic studies to estimate reductions in CVD resulting from lower salt intake. They calculated the average out-of-pocket (OOP) cost of CVD care and estimated the reduction in OOP expenditures and government subsidies due to the policy. They also estimated the costs of policy implementation and financial risk protection (FRP) benefits. The…
