ReviewPublication 2016Choosing an Epidemiological Model Structure for Economic Evaluation
This review presents a taxonomy of epidemiological model structures and applies it to the economic …
This review presents a taxonomy of epidemiological model structures and applies it to the economic evaluation of public health interventions for non-communicable diseases. Growing pressures on health services and on social care have led to a greater need for prevention of chronic diseases. In order for decision makers to make informed judgements about how to best spend finite public health resources, they must be able to quantify the anticipated costs, benefits, and opportunity costs of…

ArticlePublication 2013Contribution of H. Pylori and Smoking to US Incidence of Gastric Adenocarcinoma: A Microsimulation Model
Although gastric cancer has declined dramatically in the US, the disease remains the second leading …
Although gastric cancer has declined dramatically in the US, the disease remains the second leading cause of cancer mortality worldwide. This analysis estimates the contribution of risk factor trends on past and future intestinal-type non-cardia gastric adenocarcinoma (NCGA) incidence. The authors developed a population-based microsimulation model of intestinal-type NCGA and calibrated it to U.S. epidemiologic data on precancerous lesions and cancer. The model explicitly incorporated the impact of Helicobacter pylori and smoking on disease natural history, for which…

In recent years, much attention has been devoted to the study of happiness, yet Daniel Kahneman argues that there is confusion around defining happiness or well-being. He distinguishes between the happiness of our “experiencing selves” (whether we are happy in the moment) and our “remembering selves” (whether we are happy with the state of our lives on reflection). When we make choices, therefore, our decisions may be biased toward pleasing either the experiencing or remembering…

ArticlePublication 2024Hepatitis C Elimination in Rwanda: Progress, Feasibility, Economic Evaluation
This study evaluates the impact of Rwanda's national program launched in 2018 to eliminate hepatitis …
This study evaluates the impact of Rwanda's national program launched in 2018 to eliminate hepatitis C virus (HCV) and identifies strategies to achieve World Health Organization (WHO) elimination goals by 2030. Employing a microsimulation model spanning 2015 to 2050, the analysis assesses HCV epidemic trends, prevalence, mortality, and total care costs under various scenarios. Results show that between 2018 and 2022, over 7 million people were screened and 60,000 treated, projecting Rwanda's potential achievement of…

This teaching pack, curated by the Center for Health Decision Science, features videos introducing heuristics used in decision making. While these “mental shortcuts” can be useful in some circumstances, they can lead to more errors than deliberate, rational thinking. An awareness of these heuristics is useful to decision makers. This series of videos on heuristics was developed by Professor Joe Pliskin during his residency with the CHDS Media Hub led by Jake Waxman. They reflect…

ArticlePublication 2022Cost-Effectiveness of Masked Hypertension Screening and Treatment
The study assessed the health and economic outcomes of screening and treating masked hypertension in …
The study assessed the health and economic outcomes of screening and treating masked hypertension in U.S. adults using the Cardiovascular Disease (CVD) Policy Model, a microsimulation model. The model simulated 100,000 adults suspected of having masked hypertension (office blood pressure [BP] of 120–129/<80 mm Hg, not on antihypertensive medications, and without a history of CVD). Interventions included: usual care alone, usual care with ambulatory BP monitoring (ABPM), and usual care with home BP monitoring (HBPM).…

ArticlePublication 2022Duration & Cost-Effectiveness of Hepatocellular Carcinoma Surveillance in Hepatitis C Patients After Viral Eradication
This study assesses the cost-effectiveness of biannual surveillance for hepatocellular carcinoma (HCC) in patients cured …
This study assesses the cost-effectiveness of biannual surveillance for hepatocellular carcinoma (HCC) in patients cured of hepatitis C virus (HCV) using oral direct-acting antivirals (DAAs). A microsimulation model of HCC natural history in individuals with HCV-related advanced fibrosis or cirrhosis post-DAAs was developed, integrating data on HCC incidence, tumor progression, surveillance adherence, and health state costs/utilities. Biannual ultrasound and alpha-fetoprotein surveillance were compared to no surveillance over varying durations (5 years to lifetime). Results indicate…

This economic evaluation assesses the cost-effectiveness of atrial fibrillation (AF) screening strategies, particularly utilizing wrist-worn wearable devices, to prevent strokes. Conducted using a microsimulation decision-analytic model, the study spans from September 8, 2020, to May 23, 2022, incorporating 30 million simulated individuals representative of the US population aged 65 years or older. Eight screening strategies, including six employing wrist-worn devices and two using traditional methods, were compared against no screening. Results indicate that all wrist-worn…

Resource PackPublication, Teaching Resource 2020Resource Pack: Cost-Effectiveness of SSB Excise Taxes
The use of fiscal instruments, such as taxes or subsidies, to promote healthier dietary behavior …
The use of fiscal instruments, such as taxes or subsidies, to promote healthier dietary behavior has been of increasing interest in the last decade as the evidence-base builds for the health and economic consequences of obesity, overweight, and unhealthy eating. The motivation for using fiscal instruments in nutrition policy is to make the unhealthy option less affordable and less economically attractive by increasing the price via a tax, and therefore reduce the incentive to consume…
