ArticlePublication 2024Extended Cost-Effectiveness Analysis of Interventions to Improve Uptake of Diabetes Services in South Africa
This study examines the potential impact of a conditional cash transfer (CCT) program on diabetes …
This study examines the potential impact of a conditional cash transfer (CCT) program on diabetes diagnosis and treatment service utilization in South Africa (SA) using extended cost-effectiveness analysis (ECEA). Applying a Markov model over a 45-year period, the analysis compares costs, health benefits, and financial risk protection (FRP) attributes of different CCT strategies, drawing from SA-specific data. Three scenarios were simulated: covering diagnosis services only, treatment services only, and both diagnosis and treatment services. Cost-effectiveness,…

ArticlePublication 2023Out-of-Pocket Expenditures & Financial Risks Associated with Treatment of Vaccine-Preventable Diseases in Ethiopia
This study investigates out-of-pocket (OOP) expenditures and associated catastrophic health expenditures (CHEs) for vaccine-preventable diseases …
This study investigates out-of-pocket (OOP) expenditures and associated catastrophic health expenditures (CHEs) for vaccine-preventable diseases (VPDs) in Ethiopia. Through a cross-sectional costing analysis, data on OOP direct medical and nonmedical expenditures were collected from 995 households in 54 health facilities nationwide. The study focuses on VPDs in children under 5 years for pneumonia, diarrhea, measles, and pertussis, and in children under 15 years for meningitis. Mean OOP expenditures per disease episode ranged from $5·6 to…

ArticlePublication 2020Data-Driven Management of Post-Transplant Medications
Organ-transplanted patients typically receive high amounts of immunosuppressive drugs as a mechanism to reduce their …
Organ-transplanted patients typically receive high amounts of immunosuppressive drugs as a mechanism to reduce their risk of organ rejection. However, because of the diabetogenic effect of these drugs, this practice exposes them to a greater risk of new-onset diabetes after transplantation (NODAT), and hence, becoming insulin dependent. This study develops effective medication management strategies to address the common conundrum of balancing the risk of organ rejection versus that of NODAT. The article presents a robust…

ArticlePublication 2022Systematic Review of Patient Preferences, Expectations, and Values for Management and Treatment of Hypertension
This analysis summarized the evidence on the preferences, expectations, and values of hypertension management and …
This analysis summarized the evidence on the preferences, expectations, and values of hypertension management and treatment in hypertensive patients. The authors reviewed 24 studies involving 8,701 participants. Despite varying areas of focus, common themes included (1) patients often obtain hypertension information from their physicians and prefer shared patient-centered decision-making, and (2) side effects, cost, and convenience are important factors for patients when selecting a treatment regimen for hypertension.

ArticlePublication 2022Cost-Effectiveness of Masked Hypertension Screening and Treatment
The study assessed the health and economic outcomes of screening and treating masked hypertension in …
The study assessed the health and economic outcomes of screening and treating masked hypertension in U.S. adults using the Cardiovascular Disease (CVD) Policy Model, a microsimulation model. The model simulated 100,000 adults suspected of having masked hypertension (office blood pressure [BP] of 120–129/<80 mm Hg, not on antihypertensive medications, and without a history of CVD). Interventions included: usual care alone, usual care with ambulatory BP monitoring (ABPM), and usual care with home BP monitoring (HBPM).…

ArticlePublication 2019Projected U.S. State-Level Prevalence of Adult Obesity and Severe Obesity
This analysis estimates state-specific and demographic subgroup-specific trends and projections of the prevalence of categories …
This analysis estimates state-specific and demographic subgroup-specific trends and projections of the prevalence of categories of body-mass index (BMI) in the United States. Self-reported BMI from the Behavioral Risk Factor Surveillance System Survey (1993-1994 and 1999-2016) were obtained and corrected for quantile-specific self-reporting bias. Multinomial regressions were then fitted for each state and subgroup to estimate the prevalence of four BMI categories from 1990 through 2030: underweight or normal weight (BMI <25), overweight (25 to…

ArticlePublication 2021BMI-Related Healthcare Costs in the U.S.
This paper estimates continuous body mass index (BMI) related health care expenditures using data from …
This paper estimates continuous body mass index (BMI) related health care expenditures using data from the Medical Expenditure Panel Survey (MEPS) 2011-2016, adjusting BMI for self-report bias and controlling for potential confounding between BMI and medical expenditures. Costs are reported in $US 2019. The analysis found a J-shaped curve of medical expenditures by BMI, with higher costs for females and the lowest expenditures occurring at a BMI of 20.5 for adult females and 23.5 for…

ArticlePublication 2014Step-by-Step Guideline for Disease-Specific Costing Studies in LMIC
Disease-specific costing studies can be used as input into cost-effectiveness analyses and provide important information …
Disease-specific costing studies can be used as input into cost-effectiveness analyses and provide important information for efficient resource allocation. This paper provides a step-by-step guideline for conducting disease-specific costing studies in LMICs where data availability is limited and illustrates how the guideline was applied in a costing study of cardiovascular disease prevention care in rural Nigeria. The step-by-step guideline provides practical recommendations on methods and data requirements for six sequential steps: 1) definition of the study…

ArticlePublication 2022Healthcare Cost of Overweight & Obesity in South Africa
This analysis estimates the healthcare cost associated with treatment of weight-related conditions from the perspective …
This analysis estimates the healthcare cost associated with treatment of weight-related conditions from the perspective of the South African public sector payer. The authors report that the total cost of overweight and obesity is estimated to be ZAR33,194 million in 2020, representing approximately 15% of government health expenditure and equivalent to 0.67% of GDP. This analysis is an example of a bottom-up gross costing approach. The study draws South African data from multiple sources to estimate…
