There is immense interest worldwide in the notion of universal health coverage. A major policy focus in moving toward universal health coverage has been on the key policy question: what services should be made available and under what conditions? This article focuses on how a feasible set of universal health coverage services can be explicitly defined to create what is commonly known as a “health benefits package”, a set of services that can be feasibly financed…

This chapter examines the health, economic benefits, and government costs associated with scaling up a publicly financed home-based neonatal care package in rural India. This extended cost-effectiveness analysis considers two intervention scenarios against a baseline of no home-based neonatal care, both of which would utilize community health workers - one would provide coverage to approximately 7 out of 10 rural newborns, and the other would provide coverage to 9 out of 10 rural newborns.

In 2011, India introduced a home-based newborn care (HBNC) package to be delivered by community health workers across rural areas. The authors estimate the disease and economic burden that could be averted by scaling up the HBNC in rural India using IndiaSim, an agent-based simulation model. Under one scenario, the existing community health worker network begins providing HBNC for rural households without access to home- or facility-based newborn care. In the second scenario, coverage of…

ArticlePublication 2014Effect of Health-Facility Admission and Skilled Birth Attendant Coverage on Maternal Survival in India
Research in areas of low skilled attendant coverage found that maternal mortality is paradoxically higher …
Research in areas of low skilled attendant coverage found that maternal mortality is paradoxically higher in women who seek obstetric care. Using unmatched population-based case-control analysis of national datasets, the authors compared the effect of health-facility admission at any time (antenatal, intrapartum, postpartum) on maternal deaths (cases) to women reporting pregnancies (controls). Probability of maternal death decreased with increasing skilled attendant coverage, among both women who were and were not admitted to a health-facility; however,…

ArticlePublication 2013Health Benefits and Cost-Effectiveness of Strategies to Reduce Maternal Mortality in Afghanistan

This article, published in Health Policy and Planning, disaggregates data on pregnancies in Afghanistan to evaluate health outcomes and cost-effectiveness of interventions related to childbirth. These interventions include antenatal care, family planning, skilled birth attendance, access to transport, referral facilities, and quality of overall care. Outcomes include pregnancy-related complications, maternal deaths, maternal mortality ratios, costs and cost-effectiveness ratios. The authors report that increasing family planning would be the most effective individual intervention. The model suggests…

ArticlePublication 2007Including Boys in an HPV Vaccination Program: A CEA in a Low-Resource Setting
This paper looks at the cost-effectiveness of including boys vs girls alone in a pre-adolescent vaccination …
This paper looks at the cost-effectiveness of including boys vs girls alone in a pre-adolescent vaccination program against human papillomavirus (HPV) types 16 and 18 in Brazil. Using demographic, epidemiological, and cancer data from Brazil, the authors developed a dynamic transmission model of HPV infection between males and females. Model-projected reductions in HPV incidence under different vaccination scenarios were applied to a stochastic model of cervical carcinogenesis to project lifetime costs and benefits. They found that at 90%…

This paper presents a two-step approach to model calibration of a comprehensive natural history model of human papillomavirus (HPV) and cervical cancer. Using primary epidemiologic data from a longitudinal study of women in Brazil, a plausible range for each input parameter was identified that would produce model output within the 95% confidence intervals of the data. The authors then performed a simultaneous search over all input parameters to identify parameter sets that produced output consistent…

ArticlePublication 2021Alleviating the Burden of Diabetes with Health Equity Funds: Economic Evaluation of the Health & Financial Risk Protection Benefits in Cambodia

This study examines the potential distributional health and financial impacts of implementing strategies to provide financial coverage for diabetes services through Health Equity Funds (HEF) in Cambodia. Utilizing a Markov model, the trajectory of diabetes is projected over a 45-year period to estimate societal costs, health outcomes, and individual out-of-pocket expenditures associated with six HEF coverage strategies. Input parameters are derived from published literature and household survey data. Strategies covered different combinations of types of…

Extended cost-effectiveness analyses (ECEAs) build on cost-effectiveness analyses (CEAs) by assessing consequences in both the health and non-health domains. The ECEA approach proves novel in that it includes equity and non-health benefits (FRP) in the economic evaluation of health policies, which enables multiple criteria to factor in the decision-making process. More important, the ECEA approach enables the design of benefits packages, such as essential universal health care and the highest-priority package, based on the quantitative…
