This article examined the individual, communication and social determinants associated with COVID-19 vaccine uptake using national survey data collected before vaccines were available in the U.S. Of note, individuals under the federal poverty level and racial and ethnic minorities were oversampled. Outcomes included the likelihood of vaccinating self and dependents (e.g., children). Independent variables included perceptions of risk, exposure to different media for COVID-19 news, political party identification, confidence in scientists and social determinants of…

ArticlePublication 2021Health Opportunity Cost Threshold for CEA in the U.S.
Using a modeled cohort of 100,000 individuals in the United States with private health insurance, …
Using a modeled cohort of 100,000 individuals in the United States with private health insurance, the authors simulated the short-term mortality and morbidity resulting from increased premium related cancelation of insurance coverage. The authors used this model to estimate cost-effectiveness thresholds, in dollars per quality-adjusted life year (QALY) gained based on health opportunity costs. They reported the number of persons who dropped insurance coverage, resulting number of additional deaths and QALYs lost from mortality and…

The authors use a clinical example to simulate how treatment discussions can be complicated when new evidence is introduced that conflicts with existing guidelines. Even when evidence is consistent, the authors point out that current guidelines can have interpretations that don't agree with available evidence. They develop a step-wise algorithm to help guide individual clinical decisions even in the absence of general consensus related to appropriate testing and treatment.

DCP3 includes a comprehensive review of the efficacy, effectiveness, and cost-effectiveness of priority health interventions with the goal of influencing program design and resource allocation at global and country levels. DCP3 includes nine individual volumes, with the first eight structured around packages of conceptually related interventions, and the ninth providing an overview of main findings. Publicly available resources include journal articles and reports, both DCP3 and non-DCP3 related, as well as presentations, working papers, and book…

This report follows a June 2018, Forum on Microbial Threats that was held at the National Academies of Sciences, Engineering, and Medicine. This was a 1.5-day public workshop with the goal being an assessment of the current understanding of the interaction of infectious disease threats and economic activity in order to suggest future areas of research. This workshop built on prior work of the Forum and aimed to build more mutual understanding between those in…

Many low- and middle-income countries now aspire to universal health coverage, where governments ensure that all people have access to the quality health services they need without risk of impoverishment. But for universal health coverage to become reality, the health services offered must be consistent with the funds available-and this implies tough everyday choices for policymakers. This publication argues that the creation of an explicit health benefits plan-a defined list of services that are and are…

Despite the existence of effective interventions and best-practice guideline recommendations for childcare services to implement policies, practices, and programs to promote child healthy eating, physical activity, and prevent unhealthy weight gain, many services fail to do so. The primary aim of the review was to examine the effectiveness of strategies aimed to improve the implementation of policies, practices, or programs by childcare services that promote child healthy eating, physical activity, and/or obesity prevention. This article discusses three challenges of using cost-effectiveness thresholds to inform whether a third-party payer will fund a particular service. First, how is the appropriate cost-effectiveness threshold - or threshold range - to be determined? (And should there be a single threshold or multiple thresholds?) Second, how can the valuation of health benefits be refined to better capture the value of treatments to patients and to the economy as a whole? Third, how should a…
