This extended cost-effectiveness analysis evaluates public financing of HPV vaccination in China to prevent cervical cancer. Authors estimated the distribution of deaths averted by income quintile, compared vaccination paired with screening against current practice. They estimated reductions in cervical cancer incidence, net costs to the government, and patient cost savings, as well as the incremental government health care costs per death averted. When the cost per HPV vaccination is less than US$50 per vaccinated girl, vaccination was…

This chapter presents an approach for performing benefit-cost analysis using cleft lip and palate (CLP) repair as a model surgical intervention in a subspecialty hospital dedicated to CLP in India, and a benefit-cost analysis based on secondary data that model the benefit and cost of cesarean delivery for treatment of obstructed labor in 47 low- and middle-income countries. This is a review of economic evaluations of complex health interventions. Complex interventions, involving interlinked packages of care, challenge the application of current methods of economic evaluation that focus on measuring only health gain. The authors find that complex interventions may be problematic on two levels. First, the complexity means the intervention may not fit into one of the current appraisal systems, and/or second, maximizing health is not the only objective. This paper discusses the…

This study uses extended cost-effectiveness analysis (ECEA) to evaluate a hypothetical publicly financed program for rotavirus vaccination in India and Ethiopia. The authors measured program impact on: (1) averted rotavirus deaths; (2) reduction in household expenditures; (3) financial risk protection; and (4) distributional consequences across the country’s wealth strata. In India and Ethiopia, the program was predicted to decrease rotavirus deaths substantially, and effectively provide financial risk protection among the poor, while also reducing household…
