Extended cost-effectiveness analysis was developed to evaluate health interventions in terms of level and distribution of health gains and financial risk protection. This information is typically presented in a joint display format. This article develops and applies an algebraic money-metric formulation that incorporates all disaggregated outcomes and finds that ranking of health interventions is sensitive to the decision maker’s aversion to inequality across income groups and that financial risk protection gains are most important to…

Disease-specific costing studies can be used as input into cost-effectiveness analyses and provide important information for efficient resource allocation. This paper provides a step-by-step guideline for conducting disease-specific costing studies in LMICs where data availability is limited and illustrates how the guideline was applied in a costing study of cardiovascular disease prevention care in rural Nigeria. The step-by-step guideline provides practical recommendations on methods and data requirements for six sequential steps: 1) definition of the study…

This study used extended cost-effectiveness analysis to examine the impact of a 50% increase in market prices of cigarettes on health, poverty, and financial protection for men in 13 middle income countries. A 50% increase in cigarette prices would lead to about 450 million years of life gained across the 13 countries from smoking cessation, with half of these in China. Across all countries, men in the bottom income group would gain 6.7 times more…

This article, published in BMJ Global Health, aims to estimate the magnitude and intensity of catastrophic health expenditure (CHE) and factors associated with CHE for the prevention and treatment of cardiovascular disease in hospitals in Addis Ababa. Seeking prevention and treatment services for cardiovascular disease in Addis Ababa was found to pose substantial financial burden on households, affecting the poorest and those who reside outside Addis Ababa more. Smoking is considered the leading risk factor for mortality among the Armenian population. The authors conducted an extended cost-effectiveness analysis (ECEA) on increases in the excise tax on cigarettes in Armenia. Based on the World Health Organization recommendations, they analyzed the impact of a 75% increase of excise tax on the retail price of cigarettes. The ECEA found large health and financial benefits to the excise tax. It averted about 88,000 premature deaths, US$63 million of OOP…

The objective of this study was to assess the cost effectiveness of a government policy combining …
The objective of this study was to assess the cost effectiveness of a government policy combining targeted industry agreements and public education to reduce sodium intake in 183 countries worldwide. To account for heterogeneity in efficacy across countries, a range of scenarios were evaluated, including 10%, 30%, 0.5 g/day, and 1.5 g/day sodium reductions achieved over 10 years. Country specific costs of a sodium reduction policy were estimated using the WHO Noncommunicable Disease Costing Tool. Country…
