This article, published in BMJ Open, presents an economic model that combines a decision tree and a Markov cohort model to investigate the long-term cost-effectiveness of collaborative care versus usual care for individuals with depression and comorbid diabetes and/or cardiovascular disease. Data from the COINCIDE trial informs the model input parameters. The COINCIDE trial is a randomized controlled trial of collaborative care versus usual care that enrolled 387 participants from 36 primary care general practices…

The question of how to evaluate lost consumer surplus in benefit−cost analyses is controversial. There are clear health benefits of regulations that curb consumption of goods with health risks, such as tobacco products and foods high in fats, calories, sugar, and sodium. Yet, if regulations cause consumers to give up goods they like, the health benefits they experience may be offset by some utility loss, which benefit−cost analyses of regulations need to take into account.…

This study sought to understand how the general public views the quality of life effects of opioid misuse and opioid use disorder on an individual and his/her spouse, measured in terms used in economic evaluations. The study design was a cross-sectional internet survey of a US population-representative respondent panel conducted December 2013-January 2014, with a total of 2054 randomly selected adults, of whom 51.1% were male. The mean individual utility ranged from 0.574 for active injection…

Cost-effectiveness analysis has traditionally been applied primarily to very specific interventions, such as drugs and diagnostics; in addition, the evidence base drawn on for evaluating such interventions is relatively good, given the medical research industry surrounding their testing. However, with increasing success in controlling infectious diseases, many of the health challenges facing countries concern broad threats to health with multiple causes, such as obesity, where the relationship between policy action and health benefit is not…

The analysis of regulations affecting addictive or habitual goods has drawn considerable controversy. Some studies have suggested that such regulations have only small welfare benefits, as consumers value these goods despite health benefits from quitting, while other studies suggest that information or behavioral problems make existing consumption decisions a poor guide to welfare evaluation. This analysis examines potential utility offsets to health benefits of regulations affecting addictive or habitual goods theoretically and empirically. The paper…

This article reports on a model-based approach estimated averted cervical cancer cases and deaths, disability-adjusted life years (DALYs) and incremental cost-effectiveness ratios (I$/DALY averted) for human papillomavirus (HPV) vaccination of young adolescent girls using population and epidemiologic data for 33 countries in Latin America and the Caribbean (LAC). The authors found that an absolute reduction in lifetime cancer risk varied between countries, depending on incidence, proportion attributable to HPV-16 and 18, and population age-structure; for…

This article presents an epidemiologic model of hepatitis C in the United States. The authors used empirical calibration of model parameters to gain insights into uncertainty in the natural history of hepatitis C and to improve future projections. The authors identified model inputs by way of a systematic review. Model simulations were conducted and model predictions were compared with epidemiologic data on infection prevalence and mortality from liver cancer. Goodness-of-fit criteria were used to identify…

ArticlePublication 2023Cost-Effectiveness of Pharmacist Prescribing for Managing Hypertension
This study estimates the cost-effectiveness of implementing a pharmacist-prescribing intervention to improve blood pressure control …
This study estimates the cost-effectiveness of implementing a pharmacist-prescribing intervention to improve blood pressure control in the US. A cost-effectiveness analysis was conducted using a Markov model based on the pharmacist-prescribing intervention used in The Alberta Clinical Trial in Optimizing Hypertension (or RxACTION). Outcomes included cardiovascular (CV) events, end-stage kidney disease events, life years, quality-adjusted life years (QALYs), lifetime costs, and lifetime incremental cost-effectiveness ratio (ICER). Costs were based on reimbursement rates, published literature, national…

ArticlePublication 2022Systematic Review of Patient Preferences, Expectations, and Values for Management and Treatment of Hypertension
This analysis summarized the evidence on the preferences, expectations, and values of hypertension management and …
This analysis summarized the evidence on the preferences, expectations, and values of hypertension management and treatment in hypertensive patients. The authors reviewed 24 studies involving 8,701 participants. Despite varying areas of focus, common themes included (1) patients often obtain hypertension information from their physicians and prefer shared patient-centered decision-making, and (2) side effects, cost, and convenience are important factors for patients when selecting a treatment regimen for hypertension.
