This article in Value in Health evaluates the importance of improved health as compared to improved financial risk protection in the general United States population. Using a discrete-choice experiment, it finds that 31.3% of the population values cure at all costs, and 8.5% of the population use financial solvency to dominate medical decision making. This study shares insight to the US population values and trade-offs between health outcomes and financial health, and highlights the difficult…

Progress toward universal health coverage (UHC) requires making difficult trade-offs. The World Health Organization (WHO) Consultative Group on Equity and UHC has endorsed the principles for making such decisions. These principles include maximizing population health, priority for the worse off, and shielding people from health-related financial risks. But how should one apply these principles in particular cases, and how should one adjudicate between them when their demands conflict? This article by some members of the…

This resource pack on the cost-effectiveness of herpes zoster vaccination was curated to support Dr. Lisa Prosser's seminar on November 9, 2017 at the Center for Health Decision Science. Dr. Prosser discussed an economic evaluation of vaccination against herpes zoster. Herpes zoster—more commonly known as shingles—presents a major burden for older Americans but, until recently, the only available vaccine (Zoster Vaccine Live, ZVL) was relatively ineffective past 10 years. A recently approved vaccine–herpes zoster subunit…

Despite the existence of effective interventions and best-practice guideline recommendations for childcare services to implement …
Despite the existence of effective interventions and best-practice guideline recommendations for childcare services to implement policies, practices, and programs to promote child healthy eating, physical activity, and prevent unhealthy weight gain, many services fail to do so. The primary aim of the review was to examine the effectiveness of strategies aimed to improve the implementation of policies, practices, or programs by childcare services that promote child healthy eating, physical activity, and/or obesity prevention. The secondary…

There is immense interest worldwide in the notion of universal health coverage. A major policy focus in moving toward universal health coverage has been on the key policy question: what services should be made available and under what conditions? This article focuses on how a feasible set of universal health coverage services can be explicitly defined to create what is commonly known as a “health benefits package”, a set of services that can be feasibly financed…

The analysis of regulations affecting addictive or habitual goods has drawn considerable controversy. Some studies have suggested that such regulations have only small welfare benefits, as consumers value these goods despite health benefits from quitting, while other studies suggest that information or behavioral problems make existing consumption decisions a poor guide to welfare evaluation. This analysis examines potential utility offsets to health benefits of regulations affecting addictive or habitual goods theoretically and empirically. The paper…

In this article the authors consider how benefit-cost analysis can be used retrospectively to promote understanding of the impacts after a policy is implemented. Serving as a brief primer, the authors propose that analysis can be useful for identifying needed reforms as well as in improving the conduct of future prospective analyses. The major challenges relate to estimating what would have occurred in the absence of the policy and separating the effects of the policy…

This paper provides a short introduction to the use of benefit-cost analysis to assess interventions undertaken at the city or municipal level. It introduces the concepts that underlie the conduct of benefit-cost analysis, describes the major analytic components, and discusses how to tailor the analysis to the characteristics of the policy and the resources available. It concludes with a list of references for those interested in learning more.

ArticlePublication 2013Contribution of H. Pylori and Smoking to US Incidence of Gastric Adenocarcinoma: A Microsimulation Model

Although gastric cancer has declined dramatically in the US, the disease remains the second leading cause of cancer mortality worldwide. This analysis estimates the contribution of risk factor trends on past and future intestinal-type non-cardia gastric adenocarcinoma (NCGA) incidence. The authors developed a population-based microsimulation model of intestinal-type NCGA and calibrated it to U.S. epidemiologic data on precancerous lesions and cancer. The model explicitly incorporated the impact of Helicobacter pylori and smoking on disease natural history, for which…
