ArticlePublication 2023Out-of-Pocket Expenditures & Financial Risks Associated with Treatment of Vaccine-Preventable Diseases in Ethiopia
This study investigates out-of-pocket (OOP) expenditures and associated catastrophic health expenditures (CHEs) for vaccine-preventable diseases …
This study investigates out-of-pocket (OOP) expenditures and associated catastrophic health expenditures (CHEs) for vaccine-preventable diseases (VPDs) in Ethiopia. Through a cross-sectional costing analysis, data on OOP direct medical and nonmedical expenditures were collected from 995 households in 54 health facilities nationwide. The study focuses on VPDs in children under 5 years for pneumonia, diarrhea, measles, and pertussis, and in children under 15 years for meningitis. Mean OOP expenditures per disease episode ranged from $5·6 to…

ArticlePublication 2023Estimated Travel Time & Staffing Constraints to Accessing the Ethiopian Healthcare System: Two-Step Floating Catchment Area Analysis
This study investigates disparities in health care access across different income levels and geographic locations …
This study investigates disparities in health care access across different income levels and geographic locations in Ethiopia. Employing a two-step floating catchment area method, the research estimates variations in spatial access to health care and staffing levels at facilities. Average travel time from population centers is calculated and adjusted with provider-to-population ratios. Spearman's rank tests are applied to test hypotheses about the roles of travel time versus staffing in access variations. Results reveal regional disparities,…

Public spending can improve population well-being, for example, by averting or reducing poverty. This article aims to conceptualize monetary benchmarks for health sector investments oriented towards poverty alleviation in low- and lower-middle-income countries. Priority setting in low- and lower-middle-income countries could be informed by health-sector PRBs (poverty reduction benchmarks), in addition to burden of disease and cost-effectiveness considerations. The computed PRBs, expressed in dollars per poverty case averted, can possibly be viewed in a manner…

Health system strengthening (HSS) activities should accompany disease-targeting interventions in low/middle-income countries (LMICs). Economic evaluations provide information on how these types of investment might best be balanced but can be challenging. This paper conducted a systematic review to evaluate how researchers address these economic evaluation challenges. The authors searches produced 1978 studies, out of which they included 36. Most studies used data from prospective trials and calculated cost-effectiveness directly from these trial inputs, rather than…

ArticlePublication 2022Comparative Health Systems Analysis of Differences in Catastrophic Health Expenditure
The growing burden of non-communicable diseases (NCDs) in low- and middle-income countries may have implications …
The growing burden of non-communicable diseases (NCDs) in low- and middle-income countries may have implications for health system performance in the area of financial risk protection, as measured by catastrophic health expenditure (CHE). This article compares non-communicable diseases catastrophic health expenditure to the CHE cases caused by communicable diseases across health systems to examine whether: (1) disease burden and catastrophic health expenditure are linked, (2) Catastrophic health expenditures secondary to NCDs disproportionately affect wealthier households and (3) whether the drivers…

Globally, there are 370 million children receiving school meals every day. Coverage is least in low-income countries, where the need is greatest and where program costs are viewed as high in comparison with the benefits to public health alone. This article explores the policy implications of including the returns of school feeding to other sectors in an economic analysis. The authors develop an economic evaluation methodology to estimate the costs and benefits of school feeding…

Education and health are both constituents of human capital that enable people to earn higher wages and enhance capabilities. A better understanding of how these different human capabilities are produced together could point to opportunities to help jointly reduce the wide disparities in health and education across populations. This article uses nationally and regionally representative individual-level data from Demographic and Health Surveys (DHS) for 55 low- and middle-income countries to examine patterns in human capabilities…

The recent trend of hospital closures in the US has accelerated during the COVID-19 pandemic, as hospitals have experienced financial hardship from reduced patient volume and elective surgery cases, as well as the thin financial margins for treating patients with COVID-19. This trend of hospital closures is concerning for patients, healthcare providers, and policymakers. This current opinion piece describes the challenges caused by hospital closures and discusses what policymakers should know based on the existing…

This article describes a business model for the employee health plans of large firms to bypass insurers and instead use direct contracts with hospitals which have been designated as centers of excellence. The authors propose methods of machine learning and data analytics to identify efficient and effective delivery systems, with the ultimate goal of improving health care quality.
