Many important opportunities to improve health lie outside the health sector and involve improving the conditions in which we live and work: safe design and maintenance of roads, bridges, train tracks, and airports; control of environmental pollutants; occupational safety; healthy buildings; a safe and healthy food supply; safe manufacture of consumer products; a healthy social environment; and others. Faced with the overwhelming array of possibilities, U.S. decision makers need help identifying those that can contribute the…

This case study explores the experiences of three Manhattan-based hospitals during Superstorm Sandy in 2012. It focuses on decisions made by each institution, as Sandy approached, about whether to shelter-in-place or evacuate hundreds of medically fragile patients, and how each of the three hospitals took a different approach, informed by differing perceptions of risk and related factors. The case will be useful for public health students and administrators in understanding decision-making in settings of an…

Policy makers in low-income and lower-middle-income countries (LMICs) are increasingly looking to develop ‘evidence-based’ frameworks for identifying priority health interventions. This paper synthesizes and appraises the literature on methodological frameworks – which incorporate economic evaluation evidence – for the purpose of setting healthcare priorities in LMICs. A systematic search of Embase, MEDLINE, Econlit and PubMed identified 3968 articles with a further 21 articles identified through manual searching. A total of 36 papers were eligible for inclusion.…

The 2015 edition of Methods for the Economic Evaluation of Health Care Programmes provides a 'tool kit' for undertaking health economic evaluations. The report is aimed at researchers, health service professionals and policy makers without a formal economics background. The recommendations are based on the authors' own experiences and so are practical in nature. The book describes the challenges of allocating resources efficiently and fairly, including challenges in methodology and policy formation. Examples are provided.…

General frameworks of the cancer screening process are available, but none directly compare the process in detail across different organ sites. This limits the ability of medical and public health professionals to develop and evaluate coordinated screening programs that apply resources and population management strategies available for one cancer site to other sites. This paper presents a conceptual model that incorporates a single screening episode for breast, cervical, and colorectal cancers into a unified framework based…

This paper provides a short introduction to the use of benefit-cost analysis to assess interventions undertaken at the city or municipal level. Medicare currently pays for 23 preventive services in its benefits package, the majority of which were added since 2005. In the past decade, the program has transformed from one essentially administering treatment claims, to one increasingly focused on health promotion and maintenance. What is largely unappreciated is the role cost-effectiveness analysis has played in the coverage of preventive services. This study reviews the role of cost-effectiveness analysis in Medicare coverage of preventive services and contrast it…
