The authors examine the complexities of the vaccine enterprise, a fundamental component of preventive medicine and public health, from research and development of new vaccines to financing and reimbursement of immunization services. The current climate, socially, economically and politically, presents challenges and opportunities to the U.S. to strengthen the existing systems of developing, manufacturing, regulating, distributing, funding, and administering safe and effective vaccines for all people. This report on the opioid public health crisis was released by the White House Council …
This report on the opioid public health crisis was released by the White House Council on Economic Advisors (CEA) in November 2017. It corrects previous estimates of related costs by adding the value of the associated deaths. Earlier estimates focused on medical and other expenditures, while the new report also includes estimates of the value that individuals place on reducing their own risks of premature mortality. The report notes that, in 2015, over 33,000 Americans…

This article aimed to compare the health and economic impact of 3 services recommended by …
This article aimed to compare the health and economic impact of 3 services recommended by the US Preventive Services Task Force for the primary prevention of cardiovascular disease (CVD): (1) aspirin counseling for the primary prevention of CVD and colorectal cancer, (2) screening and treatment for lipid disorders (usually high cholesterol), and (3) screening and treatment for hypertension. A microsimulation model was used to compare lifetime outcomes from the societal perspective for a US-representative birth…

The Food and Drug Administration (FDA) is required to do a regulatory impact analysis assessing the costs and benefits of its tobacco products and other regulations. This paper provides a critical review of the approach the FDA used in its proposed and final graphic warning label rule, and includes recommendations on how to improve the analysis in ways that account for the differences between tobacco use and consumption of most consumer products. To date, FDA…

ArticlePublication 2017Cost-Effectiveness of Subsidizing Fruit and Vegetable through SNAP
A diet high in fruits and vegetables is associated with reduced risk of chronic disease …
A diet high in fruits and vegetables is associated with reduced risk of chronic disease - to incentivize consumption among low-income households one proposal is to make them more affordable through the Supplemental Nutrition Assistance Program (SNAP). This cost effectiveness analysis adopts a societal perspective to estimate the value of subsidizing fruit and vegetable (FV) purchases among the one in seven Americans who participate in SNAP. A stochastic microsimulation model of obesity, type 2 diabetes, myocardial infarction,…

The U.S. Preventive Services Task Force (USPSTF) develops evidence-based recommendations about preventive care based on comprehensive systematic reviews of the best available evidence. Decision models provide a complementary, quantitative approach to support the USPSTF as it deliberates about the evidence and develops recommendations for clinical and policy use. This article describes the rationale for using modeling, an approach to selecting topics for modeling, and how modeling may inform recommendations about clinical preventive services.

This workshop report summarizes a workshop convened by the Institute of Medicine to explore the potential uses of simulation and other types of modeling for improving health. Participants worked to identify how modeling could inform population health decision making (selecting and refining potential strategies, ranging from interventions to investments) based on lessons learned from models that have been, or have not been, used successfully, opportunities and barriers to incorporating models into decision making, and data needs and…
