This article, published in Cost Effectiveness and Resource Allocation, aims to assess the cost-effectiveness of prevention and treatment of ischemic heart disease (IHD) and stroke in an Ethiopian setting. Fifteen single interventions and sixteen intervention packages are assessed from a healthcare provider perspective. The results indicate that the escalating burden of CVD and its risk factors warrants timely action. Selected CVD intervention packages could be scaled up at a modest budget increase, but the level…

ArticlePublication 2016Identifying Cost-Effective Dynamic Policies to Control Epidemics
This paper describes a mathematical decision model for identifying dynamic health policies for controlling epidemics. …
This paper describes a mathematical decision model for identifying dynamic health policies for controlling epidemics. The dynamic policies aim to select the best current intervention based on accumulating epidemic data and the availability of resources at each decision point. An algorithm is proposed to approximate dynamic policies that optimize the population's net health benefit, a performance measure which accounts for both health and monetary outcomes. The authors further illustrate how dynamic policies can be defined and…

This tutorial describes both the intent of extended cost-effectiveness analysis and its keys to allow easy implementation for health policy assessment. Extended cost-effectiveness analysis was developed to address health policy assessment, specifically to evaluate the health and financial consequences of public policies in four domains: (1) the health gains; (2) the financial risk protection benefits; (3) the total costs to the policy makers; and (4) the distributional benefits.

This is a systematic review of the thresholds used in cost-effectiveness analyses in several international settings. While many health care systems claim to incorporate the cost-effectiveness criterion in their investment decisions, the threshold value is often controversial and not publicly available. Even when available, it often lacks a theoretical or empirical basis. This article aims to identify and critically appraise the conceptual perspectives and methodologies used to date to estimate the cost-effectiveness threshold. The authors…

This companion volume to Disease Control Priorities, Third Edition (DCP3), explores the impact of noncommunicable diseases (NCDs) on development and economic growth in the countries of Latin America and the Caribbean (LAC). This collection of manuscripts examines the complex interplay among NCDs, health expenditures and financial investments in health, poverty, and inequities, using up-to-date information and evidence from the LAC region. There is compelling proof that NCDs are a major and growing problem for low- and…

Investments in cancer control-prevention, detection, diagnosis, surgery, palliative care-are needed in low-income and particularly in middle-income countries, where most of the world's cancer deaths occur without treatment or palliation. To help countries expand locally appropriate services, Disease Control Priorities, 3rd edition developed an essential package of potentially cost-effective measures for countries to consider and adapt. Interventions included in the package are: prevention of tobacco-related cancer and virus-related liver and cervical cancers; diagnosis and treatment of…

The goal of this American Journal of Preventive Medicine supplement on “The Use of Economics in Informing U.S. Public Health Policy” is to influence policy researchers to identify and undertake economic research that generates the key evidence needed to inform policy. In public health, economic evaluation, primarily cost and cost-effectiveness analysis, has been widely used to demonstrate the economic burden of health-related conditions and the value of proposed programs and policies. However, despite the wealth…

Many important opportunities to improve health lie outside the health sector and involve improving the conditions in which we live and work: safe design and maintenance of roads, bridges, train tracks, and airports; control of environmental pollutants; occupational safety; healthy buildings; a safe and healthy food supply; safe manufacture of consumer products; a healthy social environment; and others. Faced with the overwhelming array of possibilities, U.S. decision makers need help identifying those that can contribute the…

This study assesses the health and economic impact of hepatitis C virus (HCV) screening and treatment in U.S. prisons on the broader HCV epidemic. Employing an agent-based microsimulation model of HCV transmission and disease progression, data from published literature inform the analysis. The target populations include individuals in U.S. prisons and the general community over a 30-year timeframe, adopting a societal perspective. Interventions encompass risk-based and universal opt-out HCV screening in prisons, followed by treatment…
