This paper provides a short introduction to the use of benefit-cost analysis to assess interventions undertaken at the city or municipal level. It introduces the concepts that underlie the conduct of benefit-cost analysis, describes the major analytic components, and discusses how to tailor the analysis to the characteristics of the policy and the resources available. It concludes with a list of references for those interested in learning more.

Working PaperPublication 2018Valuing Mortality Risk Reductions in Global Benefit-Cost Analysis
Increasing life expectancy is a major goal of many policies implemented around the world. As …
Increasing life expectancy is a major goal of many policies implemented around the world. As a result, the value of reducing mortality risk has been extensively studied and several organizations have developed recommendations for estimating these values in benefit-cost analysis. However, both the recommendations and the underlying research primarily address high-income settings, raising questions about the extent to which the results are applicable in low- and middle-income countries. The recommendations are also diverse, reflecting differing…

This paper explores approaches for valuing nonfatal risk reductions associated with policy choices in low- and middle-income countries. The approach for valuation ideally would be based on estimates of individuals’ willingness to pay for changes in their own risks. However, high quality valuation research is not available for many nonfatal conditions even in high-income settings. Typically, two approaches are used either alone or in combination as rough proxies. The first involves applying an estimate of…

This study aimed to systematically review discrete choice experiments (DCEs) about patients’ preferences for cancer treatment and assessed the relative importance of outcome, process and cost attributes. A systematic literature review was conducted using PubMed and EMBASE to identify all DCEs investigating patients’ preferences for cancer treatment between January 2010 and April 2016. Attributes were classified into outcome, process and cost attributes, and their relative importance was assessed. A total of 28 DCEs were identified.…

The Healthcare Cost and Utilization Project (HCUP) is the most comprehensive source of hospital data in the United States, including information on in-patient care, ambulatory care, and emergency department visits. HCUP enables researchers, insurers, policymakers and others to study health care delivery and patient outcomes over time, and at the national, regional, State, and community levels. The Agency for Healthcare Research and Quality (AHRQ) provides a range of data resources in the form of online, searchable…

Howard Raiffa (1924-2016) had a profound influence on all aspects of the decision sciences and on the fields of systems analysis and operations research. He guided the introduction of the decision sciences into numerous fields such as business, medicine, public health, the environmental sciences, and law, and was instrumental in building world-recognized institutions such as the Kennedy School at Harvard and the International Institute for Applied Systems Analysis near Vienna, Austria. Policy makers in low-income and lower-middle-income countries (LMICs) are increasingly looking to develop ‘evidence-based’ frameworks for identifying priority health interventions. This paper synthesizes and appraises the literature on methodological frameworks – which incorporate economic evaluation evidence – for the purpose of setting healthcare priorities in LMICs. A systematic search of Embase, MEDLINE, Econlit and PubMed identified 3968 articles with a further 21 articles identified through manual searching. A total of 36 papers were eligible for inclusion.…

The authors conducted a systematic review of economic evaluations of COVID-19 interventions and assessed whether they incorporated non-health impacts and distributional concerns. Among the 70 articles included, more than half (56%) included at least one non-health impact, although only 21% incorporated non-economic consequences. Only 17% examined subgroups of interest. The median ICER for the entire sample was $67,000/quality-adjusted life-year (QALY) (interquartile range [IQR] $9000-$893,000/QALY). Interventions including a pharmaceutical component yielded a median ICER of $93,000/QALY (IQR…

This study indicated that news users across ten different European countries are quite concerned about misinformation in their information environment. Respondents were most likely to associate politicians, corporations, and foreign actors with misinformation. They perceived misinformation to be most common for topics like immigration, the economy, and the environment. This offered support for the increasingly more relative and politicized status of facts in people’s credibility perceptions. Yet, differences across sources and issues were relatively modest,…
