Cost-effectiveness analysis assumes a single constraint, in the form of the budget constraint, whereas in reality decision makers may be faced with numerous other constraints. The objective of this article is to develop a typology of constraints that may act as barriers to implementation of cost-effectiveness recommendations. Six categories of constraints are considered: the design of the health system; costs of implementing change; system interactions between interventions; uncertainty in estimates of costs and benefits; weak governance;…

Distrust in scientific expertise is dangerous. Opposition to vaccination with a future vaccine against SARS-CoV-2, the causal agent of COVID-19, for example, could amplify outbreaks as happened for measles in 2019. Homemade remedies and falsehoods are being shared widely on the Internet, as well as dismissals of expert advice. There is a lack of understanding about how this distrust evolves at the system level. Authors provide a map of the contention surrounding vaccines that has…

The Organisation for Economic Co-operation and Development (OECD) Health Database offers a comprehensive source of comparable statistics on health and health systems across OECD countries. These data serve to facilitate and support comparative analyses, and can be useful for researchers, educators and policy makers looking to draw lessons from international comparisons of diverse health systems. General categories of data include population demographics, health status and determinants of health, as well as data on the societal responses to health…

The Medical Expenditure Panel Survey (MEPS) is the only national data source in the U.S. measuring how Americans use and pay for medical care, health insurance, and out-of-pocket spending. Annual surveys of individuals and families, as well as their health care providers, provide data on health status, the use of medical services, charges, insurance coverage, and satisfaction with care. This workshop report summarizes a workshop convened by the Institute of Medicine to explore the potential uses of simulation and other types of modeling for improving health. Participants worked to identify how modeling could inform population health decision making (selecting and refining potential strategies, ranging from interventions to investments) based on lessons learned from models that have been, or have not been, used successfully, opportunities and barriers to incorporating models into decision making, and data needs and…

The goal of this American Journal of Preventive Medicine supplement on “The Use of Economics in Informing U.S. Public Health Policy” is to influence policy researchers to identify and undertake economic research that generates the key evidence needed to inform policy. In public health, economic evaluation, primarily cost and cost-effectiveness analysis, has been widely used to demonstrate the economic burden of health-related conditions and the value of proposed programs and policies. However, despite the wealth…

Policy makers in low-income and lower-middle-income countries (LMICs) are increasingly looking to develop ‘evidence-based’ frameworks for identifying priority health interventions. This paper synthesizes and appraises the literature on methodological frameworks – which incorporate economic evaluation evidence – for the purpose of setting healthcare priorities in LMICs. A systematic search of Embase, MEDLINE, Econlit and PubMed identified 3968 articles with a further 21 articles identified through manual searching. A total of 36 papers were eligible for inclusion.…

Medicare currently pays for 23 preventive services in its benefits package, the majority of which were added since 2005. In the past decade, the program has transformed from one essentially administering treatment claims, to one increasingly focused on health promotion and maintenance. What is largely unappreciated is the role cost-effectiveness analysis has played in the coverage of preventive services. This study reviews the role of cost-effectiveness analysis in Medicare coverage of preventive services and contrast it…
