This resource pack, curated by the Center for Health Decision Science, includes methods papers, case studies, and reference case guidelines for conducting benefit-cost analyses in low- and middle-income countries. These resources will aid advanced students and experienced practitioners in preparing these analyses. Investing in global health and development requires making difficult choices about what initiatives to fund and what level of resources to devote to each initiative. Although benefit-cost analysis is a well-established and widely…

The authors used a systematic review of the literature to identify economic evaluations of rotavirus vaccine in LMICs and assess the extent to which equity was considered in the objectives, analysis, and results. They used equity-related indicators provided in the Guidance on Priority Setting in Health Care (GPS-Health) checklist criteria resulting in 18 unique indicators tracked. The authors found that some articles incorporated the indicators in their model inputs (20%) while the majority (80%) presented…

This paper explores approaches for valuing nonfatal risk reductions associated with policy choices in low- and middle-income countries. The approach for valuation ideally would be based on estimates of individuals’ willingness to pay for changes in their own risks. However, high quality valuation research is not available for many nonfatal conditions even in high-income settings. Typically, two approaches are used either alone or in combination as rough proxies. The first involves applying an estimate of…

This study uses extended cost effectiveness analysis (ECEA) to analyze the impacts of universal public finance (UPF) on epilepsy, schizophrenia, and depression in India and Ethiopia. The Ethiopian government has launched a National Mental Health Strategy which explicitly recognizes the importance of an efficient, equitable scale-up of mental health care within a broader, ongoing effort to increase levels of health insurance in the general population. The inclusion of universal health coverage (UHC) as a target in the health Sustainable Development Goals speaks to its importance as both a foundational and an end goal for global health. Across the globe, countries are in varying stages of progress toward UHC. To help countries where there is a wide gap between the reality of limited access and the aspirations of universality, the author argues that there must be prioritized investments to progressively realize…

This chapter examines universal public finance of the prevention and treatment of pneumonia and diarrhea in Ethiopia, with a focus on children under age five years. This extended cost-effectiveness analysis examines benefits by income quintile so that policy makers can better understand how each package affects different segments of the population and permits the incorporation of financial risk protection in the economic evaluation of health policies - both critical elements of universal health coverage.

Using extended cost-effectiveness analysis (ECEA), this paper evaluates the health and economic implications of different vaccine delivery strategies in Ethiopia: (1) routine immunization, (2) routine immunization with financial incentives, and (3) mass campaigns, known as supplemental immunization activities (SIAs), for measles vaccination. At higher costs, SIAs reached higher levels of vaccine coverage. Routine immunization paired with financial incentives was found to increase the demand among poorer households.
