Policy makers in low-income and lower-middle-income countries (LMICs) are increasingly looking to develop ‘evidence-based’ frameworks for identifying priority health interventions. This paper synthesizes and appraises the literature on methodological frameworks – which incorporate economic evaluation evidence – for the purpose of setting healthcare priorities in LMICs. A systematic search of Embase, MEDLINE, Econlit and PubMed identified 3968 articles with a further 21 articles identified through manual searching. A total of 36 papers were eligible for inclusion.…

This is a review of economic evaluations of complex health interventions. Complex interventions, involving interlinked packages of care, challenge the application of current methods of economic evaluation that focus on measuring only health gain. The authors find that complex interventions may be problematic on two levels. First, the complexity means the intervention may not fit into one of the current appraisal systems, and/or second, maximizing health is not the only objective. This paper discusses the…

COVID-19-related policies are fraught with trade-offs. Many of these trade-offs involve dimensions that can be quantitatively weighed using economic evaluation, such as those between health and cost outcomes. Other types of dimensions, such as those involving equity or autonomy, can be harder to quantify but should be considered in a comprehensive health policy decision-making context nonetheless. The authors of this New England Journal of Medicine Perspectives article outline how methods of economic evaluation and decision…

Social media is widely used as a source of health information for the general public. The potential for information shared through social media to influence health outcomes necessitates action by social media platforms to enhance access and exposure to high-quality, science-based information. This paper summarizes the work of an independent advisory group convened by the National Academy of Medicine that deliberated and gathered information to develop a set of initial principles and attributes that could…

Previous research indicated that corrective information can sometimes provoke a so-called “backfire effect” in which respondents more strongly endorsed a misperception about a controversial political or scientific issue when their beliefs or predispositions were challenged. This article shows how subsequent research and media coverage seized on this finding, distorting its generality and exaggerating its role relative to other factors in explaining the durability of political misperceptions. To the contrary, an emerging research consensus finds that…

This article presents a framework for understanding how misinformation shapes decision-making, which has cognitive representations of gist at its core. The author discusses how the framework goes beyond prior work, and how it can be implemented so that valid scientific messages are more likely to be effective, remembered, and shared through social media, while misinformation is resisted. The distinction between mental representations of the rote facts of a message – its verbatim representation – and…

U.S. Surgeon General Vivek Murthy issued a public advisory on health misinformation, calling it a “serious threat to public health” and encouraging all Americans to help slow its spread during the COVID-19 pandemic and beyond. To that end, the National Academies have been addressing misinformation in health and science on multiple fronts and are taking steps to help cultivate a fact- and evidence-based information environment. In most countries, there is an increase in ‘‘vaccine hesitancy,” defined as the proportion of parents who refuse some, many, or all vaccines or who adopt a delayed vaccination schedule, regardless of the availability of vaccination services. Generally, approaches to address vaccine hesitancy suggest either strong policies of mandatory immunization or more traditional approaches of education and information. Although mandatory immunization policies may increase uptake of vaccines, they do not address the root causes of…

The global spread of COVID-19 is affected by the spread of related misinformation – the so-called COVID-19 infodemic – that makes populations more vulnerable to the disease through resistance to mitigation efforts. This article analyzes the prevalence and diffusion of links to low-credibility content about the COVID-19 pandemic across Twitter and Facebook. They characterize cross-platform similarities and differences in popular sources, diffusion patterns, influencers, coordination, and automation. Comparing the two platforms, authors found divergence among…
