This chapter reviews the main scientific methods for guiding the allocation of resources to health: cost-effectiveness analysis (CEA) and cost-benefit analysis (CBA), sketches their methodological progress over the last several decades, and presents examples of how medical practice in other high-income countries, where people live longer, follows the priorities indicated by cost-effectiveness analysis.

ArticlePublication 2015Cost-Effectiveness & Budget Impact of Hepatitis C Virus Treatment with Sofosbuvir & Ledipasvir in the U.S.
This study evaluates the cost-effectiveness and budget impact of sofosbuvir and ledipasvir for treating chronic …
This study evaluates the cost-effectiveness and budget impact of sofosbuvir and ledipasvir for treating chronic hepatitis C virus (HCV) infection compared to the old standard of care (oSOC). Using a microsimulation model based on HCV natural history, data from published literature, and a third-party payer perspective, the analysis considers treatment-naive and treatment-experienced HCV populations in the United States over a lifetime horizon. Sofosbuvir-based therapies are found to add 0.56 quality-adjusted life-years (QALYs) relative to oSOC,…

ArticlePublication 2015Publicly Financed HPV Vaccination in China: Extended Cost-Effectiveness Analysis
This extended cost-effectiveness analysis evaluates public financing of HPV vaccination in China to prevent cervical cancer. Authors …
This extended cost-effectiveness analysis evaluates public financing of HPV vaccination in China to prevent cervical cancer. Authors estimated the distribution of deaths averted by income quintile, compared vaccination paired with screening against current practice. They estimated reductions in cervical cancer incidence, net costs to the government, and patient cost savings, as well as the incremental government health care costs per death averted. When the cost per HPV vaccination is less than US$50 per vaccinated girl, vaccination was…

Lesson/ModulePublication, Teaching Resource 2015CDC Science Ambassador Workshop 2015 Lesson Plan: Seasonal Flu Costs How Much?!
This lesson plan from the Centers for Disease Control and Prevention (CDC) was curated to …
This lesson plan from the Centers for Disease Control and Prevention (CDC) was curated to teach high school students, grades 9-10, to explore decision-making about the seasonal influenza (flu) vaccination. The lesson looks at financial decisions around vaccinations, based on a case study exploring the experiences of 282 children from cities around the United States. It looks at a wide range of factors including treatment for children with the flu and the cost of parents…

ArticlePublication 2015Health and Social Protection Effects of Measles Vaccination in Ethiopia: Extended CEA
Using extended cost-effectiveness analysis (ECEA), this paper evaluates the health and economic implications of different …
Using extended cost-effectiveness analysis (ECEA), this paper evaluates the health and economic implications of different vaccine delivery strategies in Ethiopia: (1) routine immunization, (2) routine immunization with financial incentives, and (3) mass campaigns, known as supplemental immunization activities (SIAs), for measles vaccination. At higher costs, SIAs reached higher levels of vaccine coverage. Routine immunization paired with financial incentives was found to increase the demand among poorer households.

ArticlePublication 2014Effect of Health-Facility Admission and Skilled Birth Attendant Coverage on Maternal Survival in India
Research in areas of low skilled attendant coverage found that maternal mortality is paradoxically higher …
Research in areas of low skilled attendant coverage found that maternal mortality is paradoxically higher in women who seek obstetric care. Using unmatched population-based case-control analysis of national datasets, the authors compared the effect of health-facility admission at any time (antenatal, intrapartum, postpartum) on maternal deaths (cases) to women reporting pregnancies (controls). Probability of maternal death decreased with increasing skilled attendant coverage, among both women who were and were not admitted to a health-facility; however,…

ArticlePublication 2014Cost of a Primary Care-Based Childhood Obesity Prevention Intervention
This study evaluated the cost of a primary care-based obesity prevention intervention (High Five for …
This study evaluated the cost of a primary care-based obesity prevention intervention (High Five for Kids) for children ages 2-6 years, compared to usual care. U.S. pediatric guidelines recommend that childhood obesity counseling be done in primary care settings. The clinical trial aimed to modify children’s nutrition and TV viewing habits through a motivational interviewing intervention. The authors assessed the visit-related costs for children enrolled in the trial, and found that the mean costs for…

The National Institute for Health and Care Excellence (NICE, or the Institute) provides guidance to the NHS in England on the clinical and cost effectiveness of selected new and established technologies. The Institute undertakes appraisals of health technologies at the request of the Department of Health. Guidance produced by the Institute on health technologies is also applied selectively in Northern Ireland, Scotland and Wales. The purpose of this document is to provide an overview of…

The purpose of this review is to provide guidance for determining when incorporating a decision-analytic model alongside a systemic review would be of added value for decision making purposes. The purpose of systematic reviews is to synthesize the current scientific literature on a particular topic in the form of evidence reports and technology assessments to assist public and private organizations in developing strategies that improve the quality of health care and decision making. However, there…
