This tutorial illustrates the basic steps needed to develop decision trees in Amua using a disease screening example. It details the process of how to build the structure of a decision tree, parameterize the model with probabilities and relevant outcomes (i.e., life expectancy), evaluate three alternative screening strategies in a baseline scenario, and perform one-way sensitivity analyses to assess the robustness of the results to different parameter values. Amua, the Swahili word meaning “decide”/“solve”, is…

This paper investigates the cost-effectiveness of using rapid point-of-care tests for prenatal syphilis screening among pregnant women in sub-Saharan Africa, a region with syphilis prevalence rates as high as 17%, and where traditional multi-test screening methods have been challenging to implement. Focusing on newly available rapid point-of-care screening tests, strategies differed by the initial test [rapid plasma reagin (RPR), immunochromographic strip (ICS)], need for confirmation with Treponema pallidum hemagglutination assay, and number of visits required.…

Antibody testing can determine previous exposure to SARS-CoV-2 virus. Recently, the UK government has made antibody testing available to anyone wanting it, even if there is no clinical indication. The purpose of this article is to provide guidance for when to consider antibody testing in individuals with and without symptoms suggestive of current or past SARS-CoV-2 infection. Key points made by the authors include: (1) antibody testing is likely to be most useful 2 weeks…

This article reviews the implications of increased testing for COVID-19 using reverse transcriptase polymerase chain reaction (rRT-PCR) through the application of Bayes’ Theorem for three hypothetical, stylized case scenarios. The scenarios involve three patients with a low, moderate, and high pre-test probability of COVID-19 infection. The category of low probability would include "asymptomatic individuals in a presumed low prevalence environment" and might vary from 10 to 20%. The category of moderate probability would include "individuals…

This resource pack on childhood obesity was curated by the Center for Health Decision Science to showcase existing cost-effectiveness analyses and motivate students, educators, and others to pursue new applications of decision science methods to the public health challenge of obesity. The resource pack was motivated by the NEJM article entitled Simulation of Growth Trajectories of Childhood Obesity into Adulthood published on November 30, 2017, with CHDS co-authors Zach Ward and Stephen Resch. Citation: Ward Z, Long M,…

This article is part of a 6-part series on clinical decision making. The authors use two clinical examples to review the principles of interpreting diagnostic test results. They outline an approach that can be used to determine how to select and apply tests and their results to the practice of internal medicine. Topics covered in the two case studies include sensitivity and specificity, positive predictive and negative predictive value of tests, and how to estimate…

This report from the World Bank is the first volume of the Disease Control Priorities, Third Edition (DCP3) series. Essential Surgery presents data on the surgical burden of disease, disability, congenital abnormalities, and trauma, as well as the health impact and economic analysis of procedures. This report identifies 44 procedures that address substantial needs, are cost effective, and are feasible to implement in low- and middle-income countries. If made universally available, these 44 procedures could…

This chapter is part of a report entitled, Decision and Simulation Modeling in Systematic Reviews, that seeks to provide guidance for determining when incorporating a decision-analytic model alongside a systemic review would be of added value for decision making purposes. The chapter discusses the role of decision analysis and decision-analytic models in health care, specifically within the context of the current emphasis on evidence-based medicine and the proliferation of systematic reviews. It describes the types of model available…

In 2016 the World Health Assembly adopted the Global Strategy on Sexually Transmitted Infections (STI) 2016–2021 aiming to reduce curable STIs by 90% by 2030. This analysis costed scaling-up priority interventions to achieve coverage targets. Strategy-targeted declines in Chlamydia trachomatis, Neisseria gonorrhoeae, Treponema pallidum and Trichomonas vaginalis were applied to WHO-estimated regional burdens at 2012 levels. Case management was costed for the curable STIs, symptomatic Herpes Simplex Virus 2 (HSV-2), and non-STI vaginal syndromes, with incrementally expanding diagnoses. Service…
