Smoking is considered the leading risk factor for mortality among the Armenian population. The authors conducted an extended cost-effectiveness analysis (ECEA) on increases in the excise tax on cigarettes in Armenia. Based on the World Health Organization recommendations, they analyzed the impact of a 75% increase of excise tax on the retail price of cigarettes. The ECEA found large health and financial benefits to the excise tax. It averted about 88,000 premature deaths, US$63 million of OOP…

In this paper, the authors examine the use of solid fuels as a primary energy source for cooking in India, which contributes to high rates of infant and child mortality as well as other diseases caused by household air pollution (HAP). To achieve the widespread adoption of one of three interventions – a mud chimney stove, a blower stove, and LPG use—the government needs to offer subsidies to households using solid fuels. While the reduction…

The objective of this study was to assess the cost effectiveness of a government policy combining targeted industry agreements and public education to reduce sodium intake in 183 countries worldwide. To account for heterogeneity in efficacy across countries, a range of scenarios were evaluated, including 10%, 30%, 0.5 g/day, and 1.5 g/day sodium reductions achieved over 10 years. Country specific costs of a sodium reduction policy were estimated using the WHO Noncommunicable Disease Costing Tool. Country…

The Medical Expenditure Panel Survey (MEPS) is the only national data source in the U.S. measuring how Americans use and pay for medical care, health insurance, and out-of-pocket spending. Annual surveys of individuals and families, as well as their health care providers, provide data on health status, the use of medical services, charges, insurance coverage, and satisfaction with care. The Agency for Healthcare Research and Quality (AHRQ) provides a range of data resources in the form…

The goal of this American Journal of Preventive Medicine supplement on “The Use of Economics in Informing U.S. Public Health Policy” is to influence policy researchers to identify and undertake economic research that generates the key evidence needed to inform policy. In public health, economic evaluation, primarily cost and cost-effectiveness analysis, has been widely used to demonstrate the economic burden of health-related conditions and the value of proposed programs and policies. However, despite the wealth…

This paper evaluates the potential health and financial risk protection effects of a policy of universal public finance (UPF) to treating schizophrenia in India. The study uses the extended cost effectiveness analysis framework across income quintiles. The results show financial protection benefits concentrated in the richest income quintiles, while health gains were concentrated among the poorest. Cost-effectiveness analysis assumes a single constraint, in the form of the budget constraint, whereas in reality decision makers may be faced with numerous other constraints. The objective of this article is to develop a typology of constraints that may act as barriers to implementation of cost-effectiveness recommendations. Six categories of constraints are considered: the design of the health system; costs of implementing change; system interactions between interventions; uncertainty in estimates of costs and benefits; weak governance;…

The analysis of regulations affecting addictive or habitual goods has drawn considerable controversy. Some studies have suggested that such regulations have only small welfare benefits, as consumers value these goods despite health benefits from quitting, while other studies suggest that information or behavioral problems make existing consumption decisions a poor guide to welfare evaluation. This analysis examines potential utility offsets to health benefits of regulations affecting addictive or habitual goods theoretically and empirically. The paper…
