Part of a six-part series of articles on clinical decision making, in this article, the authors discuss how to incorporate perspective into clinical decisions, explicitly acknowledging that the treating physician is not the only stakeholder in these decisions. The authors use 2 case studies to demonstrate how changes in perspective can alter the clinical decision as well lead to both intended and unintended consequences to the outcomes.

Rising rates of obesity represent one of the greatest public health threats facing the United States. Obesity has been linked to excess consumption of sugary drinks. Federal, state, and local governments have considered implementing excise taxes on sugary drinks to reduce consumption, reduce obesity, and provide a new source of government revenue. This resource pack includes a series of briefs describing analyses conducted by the Childhood Obesity Intervention Cost-Effectiveness Study (CHOICES), evaluating the health and economic impact…

The authors use a clinical example to simulate how treatment discussions can be complicated when new evidence is introduced that conflicts with existing guidelines. Even when evidence is consistent, the authors point out that current guidelines can have interpretations that don't agree with available evidence. They develop a step-wise algorithm to help guide individual clinical decisions even in the absence of general consensus related to appropriate testing and treatment.

This article is part of a 6-part series on clinical decision making. The authors use two clinical examples to review the principles of interpreting diagnostic test results. They outline an approach that can be used to determine how to select and apply tests and their results to the practice of internal medicine. Topics covered in the two case studies include sensitivity and specificity, positive predictive and negative predictive value of tests, and how to estimate…

This article evaluates the potential impact of rolling out the Ying Yang Bao (YYB) nutritional package on child stunting across provinces and wealth groups in China, focusing on equity. Utilizing data from the China Family Panel Studies and extended cost-effectiveness analysis methods, the study estimates the distributional impact of a 12-month YYB program for children aged 6-36 months across 25 provinces and two wealth groups. Results indicate that a 75% coverage of YYB could avert…

This chapter is part of a report entitled, Decision and Simulation Modeling in Systematic Reviews, that seeks to provide guidance for determining when incorporating a decision-analytic model alongside a systemic review would be of added value for decision making purposes. The chapter discusses the role of decision analysis and decision-analytic models in health care, specifically within the context of the current emphasis on evidence-based medicine and the proliferation of systematic reviews. It describes the types of model available…

In this teaching pack on Using Test Information I, students review the performance of a dichotomous test and the relationship between sensitivity and specificity, calculate likelihood ratios to describe test performance, and conduct probability revision using the odds-LR form of Bayes. Materials include an instructor's note, videos, companion slides, a glossary, an annotated bibliography, and sample exercises. Learning Objectives Calculate four conditional probabilities describing the performance of a dichotomous test, and explain the…
