ArticlePublication 2022Child Health Inequity through Case Management of Under-Five Malaria in Nigeria: An ECEA
This study assesses the potential impact of subsidies covering the direct and indirect costs of …
This study assesses the potential impact of subsidies covering the direct and indirect costs of under-five malaria case management in Nigeria, utilizing an extended cost-effectiveness analysis (ECEA) and a decision tree model. Findings reveal that fully subsidizing medical, non-medical, and indirect costs could annually avert over 19,000 under-five deaths, 8,600 cases of catastrophic health expenditure (CHE), and US$187 million in out-of-pocket (OOP) spending. Per US$1 million invested, this translates to a significant reduction in under-five…

ArticlePublication 2021Cost-Effectiveness of Hypertension Treatment by Pharmacists in Black Barbershops
The Los Angeles Barbershop Blood Pressure Study (LABBPS) examined the effectiveness and cost of a …
The Los Angeles Barbershop Blood Pressure Study (LABBPS) examined the effectiveness and cost of a one-year pharmacist-led hypertension care intervention in Black-owned barbershops in Los Angeles County, focused on non-Hispanic Black men with uncontrolled hypertension. Using a discrete event simulation, the researchers projected the 10-year health outcomes and health care costs associated with the intervention compared to a control group. The costs and quality-adjusted life-years (QALYs) were calculated from a health care sector perspective, with…

ReviewPublication 2021Users' Guide to Medical Decision Analysis
The article emphasizes the significance of decision analysis in aiding clinicians to make evidence-based choices …
The article emphasizes the significance of decision analysis in aiding clinicians to make evidence-based choices between testing and treatment strategies. While clinicians may not conduct decision analyses themselves, they often rely on publications or guidelines derived from them. The review outlines the structure of a decision tree and equips clinicians with the skills to critically evaluate decision analyses and implement their findings in medical decision-making. It highlights two critical judgments clinicians can make: first, assessing…

Antibody testing can determine previous exposure to SARS-CoV-2 virus. Recently, the UK government has made antibody testing available to anyone wanting it, even if there is no clinical indication. The purpose of this article is to provide guidance for when to consider antibody testing in individuals with and without symptoms suggestive of current or past SARS-CoV-2 infection. Key points made by the authors include: (1) antibody testing is likely to be most useful 2 weeks…

Part of a six-part series of articles on clinical decision making, in this article, the authors discuss how to incorporate perspective into clinical decisions, explicitly acknowledging that the treating physician is not the only stakeholder in these decisions. The authors use 2 case studies to demonstrate how changes in perspective can alter the clinical decision as well lead to both intended and unintended consequences to the outcomes.

ArticlePublication 2020Equity & Distributional Impact on Stunting of a Nutritional Package, Children Aged 6-36 Months in China: Modeling Study Findings
This article evaluates the potential impact of rolling out the Ying Yang Bao (YYB) nutritional …
This article evaluates the potential impact of rolling out the Ying Yang Bao (YYB) nutritional package on child stunting across provinces and wealth groups in China, focusing on equity. Utilizing data from the China Family Panel Studies and extended cost-effectiveness analysis methods, the study estimates the distributional impact of a 12-month YYB program for children aged 6-36 months across 25 provinces and two wealth groups. Results indicate that a 75% coverage of YYB could avert…

This study uses the extended cost effectiveness analysis (ECEA) to evaluate the impact of immunization against measles, severe pneumococcal disease and severe rotavirus for birth cohorts vaccinated over 2016–2030 for three scenarios in 41 Gavi-eligible countries: no immunization, current immunization coverage forecasts and the current immunization coverage enhanced with funding support. Following the distribution of the cases by socioeconomic group, the study found that the number of catastrophic health costs (CHC) cases attributable to measles,…

This study used extended cost-effectiveness analyses to estimate the effect of the sweetened beverages tax …
This study used extended cost-effectiveness analyses to estimate the effect of the sweetened beverages tax in the Philippines on the numbers of premature deaths averted attributed to type 2 diabetes mellitus, ischemic heart disease and stroke, across income quintiles over the period 2018-2037. The study also estimated the financial benefits of the tax from reductions in out-of-pocket payments, direct medical costs averted, and government health-care cost savings. The analysis showed that the tax could avert…

This study compared three methods of blood pressure (BP) measurement for diagnosing hypertension in primary care settings, accounting for the possibility of false-positive (white-coat hypertension) and false-negative (masked hypertension) clinic measurements. Outcomes included quality-adjusted life years (QALYs) and lifetime costs associated with clinic BP measurement, home BP monitoring, and ambulatory blood pressure monitoring (ABPM) under two scenarios: positive and negative initial screening. Data were from published literature, public data sources, and expert input. In the…
