This report from RAND reviews decision-support tools, including models and nonmodeling approaches, that are relevant to infectious disease prevention, detection, and response and aligns these tools with real-world policy questions that the tools can help address. This overview is designed to help modelers and other technical experts understand the questions that policymakers will raise and the decisions they must make. The report also presents policymakers with the capabilities and limitations of the different tools that may…

ArticlePublication 2018Equity Impact Vaccines May Have on Averting Deaths and Medical Impoverishment

In this analysis, authors estimated the number of deaths averted and the number of cases of medical impoverishment averted of ten antigens and their corresponding vaccines across income quintiles for forty-one low- and middle-income countries. The study found that vaccines administered between 2016 and 2030 would prevent 36 million deaths. Vaccines will have the greatest impact on reducing cases of poverty caused by hepatitis B, helping an estimated 14 million people avoid medical impoverishment. An…

Using extended cost-effectiveness analysis (ECEA), this paper evaluates the health and economic implications of different vaccine delivery strategies in Ethiopia: (1) routine immunization, (2) routine immunization with financial incentives, and (3) mass campaigns, known as supplemental immunization activities (SIAs), for measles vaccination. At higher costs, SIAs reached higher levels of vaccine coverage. Routine immunization paired with financial incentives was found to increase the demand among poorer households.

High out-of-pocket medical expenses for cervical cancer can lead to catastrophic health expenditures and medical impoverishment in many low-resource settings. This article uses a static cohort model that captures the main features of HPV vaccines and population demographics to project health and economic outcomes associated with routine HPV vaccination in Ethiopia. The findings show that, assuming 100% vaccine efficacy against HPV-16/18 and 50% vaccination coverage, routine HPV vaccination could avert up to 970 000 cases…

This modelling study provides estimates of global childhood cancer survival, accounting for the impact of …
This modelling study provides estimates of global childhood cancer survival, accounting for the impact of multiple factors that affect cancer outcomes in children. The authors developed a microsimulation model to simulate childhood cancer survival for 200 countries/territories, accounting for clinical and epidemiologic factors, including country-specific treatment variables, such as availability of chemotherapy, radiation, and surgery, and calibrated the model to empirical data from the CONCORD-2 and CONCORD-3 studies using an Approximate Bayesian Computation approach. The…

ArticlePublication 2021Measuring the Impact of COVID-19 Vaccine Misinformation on Vaccination Intent in the U.K. and U.S.

Widespread acceptance of a vaccine for SARS-CoV-2 will be the next major step to fight the COVID-19 pandemic, but high uptake achievement will be a challenge and may be impeded by online misinformation. This article is a randomized controlled trial in the U.K. and the U.S. with the objective to quantify how exposure to online misinformation around COVID-19 vaccines affects intent to vaccinate to protect oneself or others. The authors report that in both countries…

People's inability to update their memories in light of corrective information may have important public health consequences, as in the case of vaccination choice. The authors compare three potentially effective strategies in vaccine promotion: (1) one contrasting myths vs. facts, (2) one employing fact and icon boxes, and (3) one showing images of non-vaccinated sick children. Beliefs in the autism/vaccines link and in vaccines side effects, along with intention to vaccinate a future child, were…

Vaccination is one of the great achievements of the 20th century, yet persistent public health problems include inadequate, delayed, and unstable vaccination uptake. Psychology offers three general propositions for understanding and intervening to increase uptake where vaccines are available and affordable. The first proposition is that thoughts and feelings can motivate getting vaccinated. Hundreds of studies have shown that risk beliefs and anticipated regret about infectious disease correlate reliably with getting vaccinated; low confidence in…

The internet is often praised as a tool for freedom of speech, democracy, and truth. However, the internet increasingly has become polluted by misinformation – the inadvertent spread of misleading and false information – and disinformation – the deliberate and coordinated spread of misleading and false information. Individuals online knowingly and unknowingly spread dangerous rumors and propaganda at an alarming rate, which can mislead or manipulate the worldview of those who encounter it. False information…
