This paper compares alternative strategies to manage ductal carcinoma in situ (DCIS). The authors used a disease simulation model to simulate the clinical events after six treatments (lumpectomy alone, lumpectomy with radiation, lumpectomy with radiation and tamoxifen, lumpectomy with tamoxifen, and mastectomy with and without breast reconstruction). Outcomes included disease-free, invasive disease-free, overall survival and breast preservation. Data were from the published literature. The results showed that for women aged 45 years at diagnosis, both mastectomy…

ArticlePublication 2021CEA of Alternative Colorectal Cancer Screening Strategies in High-Risk Individuals
Fecal occult blood testing with an immunochemical test (FIT) is generally considered as the most …
Fecal occult blood testing with an immunochemical test (FIT) is generally considered as the most cost-effective alternative in colorectal cancer screening programs for average risk individuals without family history. Colorectal screening guidelines recommend colonoscopy every 3-5 years for high-risk individuals with a family history. The authors use a microsimulation model of the natural history of colorectal cancer (CRC) to compare the costs, quality-adjusted life years, and cost effectiveness associated with several screening strategies in high-risk individuals,…

Antibody testing can determine previous exposure to SARS-CoV-2 virus. Recently, the UK government has made antibody testing available to anyone wanting it, even if there is no clinical indication. The purpose of this article is to provide guidance for when to consider antibody testing in individuals with and without symptoms suggestive of current or past SARS-CoV-2 infection. Key points made by the authors include: (1) antibody testing is likely to be most useful 2 weeks…

Antibody testing can determine previous exposure to SARS-CoV-2 virus. This interactive calculator, linked to the article below* on antibody testing for SARS-CoV-2 virus, allows users to vary the prior probability of infection, the sensitivity of SARS-CoV-2 antibody testing, and the specificity of SARS-CoV-2 antibody testing. Key points made in the article accompanying the interactive include: (1) antibody testing is likely to be most useful 2 weeks after infection, (2) sensitivity and specificity will vary over time and…

Part of a six-part series of articles on clinical decision making, in this article, the authors discuss how to incorporate perspective into clinical decisions, explicitly acknowledging that the treating physician is not the only stakeholder in these decisions. The authors use 2 case studies to demonstrate how changes in perspective can alter the clinical decision as well lead to both intended and unintended consequences to the outcomes.

This resource pack on childhood obesity was curated by the Center for Health Decision Science to showcase existing cost-effectiveness analyses and motivate students, educators, and others to pursue new applications of decision science methods to the public health challenge of obesity. The resource pack was motivated by the NEJM article entitled Simulation of Growth Trajectories of Childhood Obesity into Adulthood published on November 30, 2017, with CHDS co-authors Zach Ward and Stephen Resch. Citation: Ward Z, Long M,…

This analysis estimated the long-term health benefits and health care cost-savings of reductions in body mass index (BMI) for the Australian population of children aged between 2 and 5 years. A proportional multistate, multiple cohort, lifetable model estimated the health benefits and health care cost-savings related to hypothetical reductions in BMI, informed by a scoping review of systematic reviews reporting the effectiveness of obesity prevention interventions in preschool aged children. Results suggested significant potential for…

The authors use a clinical example to simulate how treatment discussions can be complicated when new evidence is introduced that conflicts with existing guidelines. Even when evidence is consistent, the authors point out that current guidelines can have interpretations that don't agree with available evidence. They develop a step-wise algorithm to help guide individual clinical decisions even in the absence of general consensus related to appropriate testing and treatment.

This article, published in BMJ Global Health, provides experiences of country contextualization of WHO-CHOICE methods and models to a country level. Results from three contextualized cost-effectiveness analyses (CEAs) are presented, and the authors discuss how this evidence can inform priority setting in Ethiopia. This method of contextualized CEAs requires inclusion of national analysts and use of country-specific inputs for either costs, epidemiology, demography, baseline coverage or effects. Rank ordering of interventions by incremental cost-effectiveness ratios…
